屏氣fMRI評價2型糖尿病患者腦血管反應(yīng)能力

【摘要】:目的利用fMRI結(jié)合屏氣方法評價2型糖尿病(T2DM)患者的腦血管反應(yīng)能力(CVR)。方法對25例T2DM患者(T2DM組)和25名健康志愿者(對照組)進行屏氣條件下的fMRI試驗。采用SPM8進行數(shù)據(jù)處理和統(tǒng)計分析,組內(nèi)分析采用單樣本t檢驗,組間分析采用雙樣本t檢驗。將存在CVR組間差異的腦區(qū)建立為ROI,提取所有受試者的BOLD信號變化百分比(PSC),比較組間ROI內(nèi)平均PSC的差異,并將T2DM組的PSC與臨床行為學(xué)、心理學(xué)及血糖控制水平進行相關(guān)分析。結(jié)果無并發(fā)癥的T2DM患者與對照組的CVR分布均以灰質(zhì)為主(P0.05,FWE校正),T2DM組雙側(cè)前額葉內(nèi)側(cè)面及背外側(cè)面的CVR明顯高于對照組(P0.01,AlphaSim校正),該區(qū)域的PSC與臨床行為學(xué)、心理學(xué)和血糖控制水平之間無相關(guān)性。結(jié)論屏氣fMRI可用于評價CVR,無明顯并發(fā)癥的T2DM患者雙側(cè)前額葉的CVR升高,可能處于代償期,有發(fā)生記憶和執(zhí)行功能障礙的風(fēng)險。
[Abstract]:Objective to evaluate the cerebrovascular response ability (CVR).) of type 2 diabetes mellitus (T2DM) patients by fMRI combined with breath-holding method. Methods fMRI test was performed on 25 T2DM patients (T2DM group) and 25 healthy volunteers (control group). SPM8 was used for data processing and statistical analysis. Single sample t-test was used for intra-group analysis and two-sample t-test was used for inter-group analysis. The brain region with difference in CVR was established as the percentage of BOLD signal change in all subjects extracted by ROI,. The average PSC in ROI was compared between the two groups by (PSC), and the correlation between PSC and clinical behavior, psychology and blood glucose control level in T2DM group was analyzed. Results the distribution of CVR in T2DM patients without complications was mainly gray matter (P0.05 FWE correction). The CVR in bilateral medial prefrontal lobe and dorsolateral side of T2DM group was significantly higher than that in control group (P0.01 AlphaSim correction). PSC and clinical behavior in this area were significantly higher than those in control group. There was no correlation between psychology and blood glucose control levels. Conclusion breath-holding fMRI can be used to evaluate the increase of CVR in bilateral prefrontal lobe of T2DM patients without obvious complications, which may be in compensatory stage and may have the risk of memory and executive dysfunction.
