【摘要】：目的：分析白塞病以心臟瓣膜損害為主要表現(xiàn)的臨床特點及超聲心動圖改變。 方法：回顧分析2006-2012年我院以心臟瓣膜損害為突出表現(xiàn)收治且診斷白塞病的21例患者的臨床資料。 結(jié)果：本組患者中男性17例,女性4例,平均年齡39.5歲。21例患者均表現(xiàn)出主動脈瓣反流,反流量中量至大量,超聲心動圖無特征性改變,多表現(xiàn)為主動脈瓣脫垂。5例非手術(shù)治療,16例手術(shù)治療。主動脈瓣位人工瓣置換22例次,術(shù)后均發(fā)生瓣周漏；1例主動脈根部置換術(shù),結(jié)合圍手術(shù)期免疫抑制治療效果良好；3例行心臟移植,術(shù)后早期心功能良好,無排異。住院期間2例分別因換瓣術(shù)后低心排、動脈瘤破裂死亡。隨訪期間有10例換瓣術(shù)后患者主動脈瓣位人工瓣瓣周仍有明顯反流。 結(jié)論：白塞病心血管系統(tǒng)受累的表現(xiàn)缺乏特異性。以心臟瓣膜損害為突出表現(xiàn)的非風濕性非先天性主動脈瓣反流者,尤其是年輕患者,應警惕為白塞病所致。
[Abstract]:Objective: to analyze the clinical features and echocardiographic changes of Behcet's disease. Methods: the clinical data of 21 patients with Behcet's disease admitted to our hospital from 2006 to 2012 were retrospectively analyzed. Results: there were 17 males and 4 females with an average age of 39.5 years. 21 patients showed aortic valve regurgitation with moderate to large volume of regurgitation and no characteristic changes in echocardiography. Most of them showed aortic valve prolapse. 5 cases were treated by non-operation and 16 cases were treated surgically. Aortic valve replacement was performed in 22 cases. Perivalvular leakage occurred in 1 case. Combined with perioperative immunosuppressive therapy, cardiac transplantation was performed in 3 cases. Cardiac function was good in the early postoperative period without rejection. Two patients died of rupture of aneurysm after valve replacement. During the follow-up period, 10 patients with aortic valve prosthesis still had significant regurgitation after valve replacement. Conclusion: the manifestation of cardiovascular system involvement in Behcet's disease is not specific. Non-rheumatic non-congenital aortic valve regurgitation, especially in young patients, should be on guard against Behcet's disease.
