【摘要】：目的探討超聲檢查多指標(biāo)積分法鑒別診斷甲狀腺良惡性小結(jié)節(jié)的臨床價值。方法 73例甲狀腺實性小結(jié)節(jié)患者,術(shù)后經(jīng)病理檢查證實良性結(jié)節(jié)44個、惡性結(jié)節(jié)52個。分析其術(shù)前超聲影像學(xué)資料,并采用超聲多指標(biāo)積分法對其二維聲像圖特征及彩色多普勒血流成像特點予以評分。結(jié)果 73例患者、96個結(jié)節(jié)經(jīng)超聲檢查診斷為惡性結(jié)節(jié)40個、良性結(jié)節(jié)56個,誤診18個,診斷符合率81.25%(78/96)。甲狀腺微小乳頭狀癌超聲聲像圖特征為形態(tài)不規(guī)則、邊界模糊、低回聲、縱橫比≥1、有微小及簇狀鈣化、內(nèi)部血流豐富。44個良性結(jié)節(jié)多指標(biāo)積分平均1.86分,52個惡性結(jié)節(jié)多指標(biāo)積分平均3.38分,二者比較差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論臨床可采用超聲檢查多指標(biāo)積分對甲狀腺良惡性小結(jié)節(jié)進(jìn)行鑒別診斷,診斷符合率較高。
[Abstract]:Objective to study the clinical value of multi-index integration in differential diagnosis of benign and malignant thyroid nodules. Methods Seventy-three patients with solid thyroid nodules were confirmed by pathological examination of 44 benign nodules and 52 malignant nodules. The ultrasonic imaging data before operation were analyzed, and the characteristics of two-dimensional sonogram and color Doppler flow imaging were evaluated by multi-index integration method. Results among 73 patients, 96 nodules were diagnosed as malignant nodules (40), benign nodules (56) and misdiagnoses (18 / 96). The diagnostic coincidence rate was 81.25% (78 / 96).
