【摘要】：目的:明確正常成人男性會(huì)陰部肌肉骨骼系統(tǒng)的超聲聲像圖特征,闡明正常成人男性會(huì)陰部肌肉骨骼系統(tǒng)的高頻超聲掃查方法。方法:使用Toshiba Aplio500型高頻超聲診斷儀檢查30例正常成人男性會(huì)陰部,通過(guò)橫切、縱切及斜切等掃查方法,記錄并描述正常成人男性會(huì)陰部肌肉、骨骼、韌帶、血管、神經(jīng)的超聲聲像圖;測(cè)量尿道球的最大橫截面積各3次,取其平均值,分別按照年齡5歲間距、身高5cm間距、體重5kg間距進(jìn)行分組,采用方差分析進(jìn)行統(tǒng)計(jì)學(xué)分析,分析年齡5歲間距組、身高5cm間距組及體重5kg間距組受檢者尿道球橫截面積有無(wú)統(tǒng)計(jì)學(xué)差異。結(jié)果:本研究以恥骨聯(lián)合、坐骨結(jié)節(jié)、尾骨尖、肛門(mén)及肛尾韌帶等為解剖標(biāo)志,獲取正常成人男性會(huì)陰部各個(gè)切面的高頻超聲聲像圖。具體描述了會(huì)陰深隙、會(huì)陰淺隙、坐骨直腸窩、陰部管、陰股溝內(nèi)容物及肛門(mén)外括約肌、肛提肌、會(huì)陰中心腱、肛提肌腱弓、坐骨海綿體肌、球海綿體肌、會(huì)陰淺橫肌、會(huì)陰深橫肌的相互毗鄰關(guān)系和超聲聲像圖特點(diǎn);闡明了陰部?jī)?nèi)動(dòng)脈、肛動(dòng)脈、陰莖球動(dòng)脈、陰莖深動(dòng)脈及陰部神經(jīng)的超聲定位及超聲聲像圖特征;明確了肛尾韌帶、恥骨弓狀韌帶的起止及其超聲聲像圖特點(diǎn)。統(tǒng)計(jì)學(xué)分析結(jié)果表明:年齡5歲間距組、身高5cm間距組及體重5kg間距組的受檢者尿道球橫截面積差異無(wú)統(tǒng)計(jì)學(xué)意義(P≥0.05)。結(jié)論:高頻超聲可作為正常成人男性會(huì)陰部肌肉骨骼系統(tǒng)首選的無(wú)創(chuàng)性影像學(xué)檢查方法,值得在臨床大力推廣。
[Abstract]:Objective:To clarify the ultrasonographic features of the perineal musculoskeletal system in normal adult males and to clarify the high-frequency ultrasonographic scanning methods of the musculoskeletal system in normal adult males.Methods:30 normal adult males were examined by Toshiba Aplio 500 high-frequency ultrasonography. Ultrasound images of perineal muscles, bones, ligaments, blood vessels and nerves were recorded and described in normal adults. The maximum cross-sectional area of the urethral bulb was measured three times each, and the mean value was taken. The patients were divided into 5-year-old interval, 5-cm height interval and 5-kg weight interval, respectively. Results: In this study, the pubic symphysis, ischial tubercle, coccygeal tip, anal and anal-caudal ligament were used as anatomical markers to obtain high-frequency ultrasound images of normal adult male perineum. The deep perineal space, superficial perineal space, and sitting were described in detail. Bone rectal fossa, pudendal canal, pudendal and thigh contents and anal external sphincter, levator ani muscle, perineal central tendon, levator ani tendon arch, ischiocavernosus muscle, bulbocavernosus muscle, superficial transverse perineal muscle, deep transverse perineal muscle adjacent relationship and ultrasonic imaging characteristics; clarified the internal pudendal artery, anal artery, penile bulbar artery, deep penile artery and pudendal nerve Statistical analysis showed that there was no significant difference in the transverse urethral area between the age 5-year-old group, the height 5 cm group and the weight 5 kg group (P < 0.05). For normal adult male perineal musculoskeletal system of the first choice of non-invasive imaging methods, it is worth promoting in clinical practice.
