【摘要】：正患者男,52歲,主因"活動(dòng)后氣短4個(gè)月,加重伴下肢水腫1周"入院。查體:雙肺無(wú)陽(yáng)性體征,叩診心界向左右兩側(cè)擴(kuò)大,心率80次/分,律齊,心尖區(qū)可聞及3/6級(jí)收縮期吹風(fēng)樣雜音,頸靜脈無(wú)怒張,肝脾肋下未及,雙下肢中度水腫。心電圖示房性纖顫。超聲:二尖瓣前葉部分腱索斷裂伴脫垂,中度反流,全心大,心尖四腔心切面可見(jiàn)右心房?jī)?nèi)約3.50cm×1.24cm分葉狀腫物,呈弱回聲,隨心動(dòng)周期左右擺動(dòng),與右心房游離壁無(wú)附著(圖
[Abstract]:The patient, 52 years old, was admitted to hospital because of "shortness of breath for 4 months after exercise, aggravated with edema of lower extremities for 1 week". Body examination: there were no positive signs in both lungs, percussion to both sides of the heart, heart rate 80 beats / min, rhythm, the apical area can be heard and 3 / 6 degree of systolic blowing murmur, jugular vein no irritation, liver and spleen without subcostal, bilateral lower extremity moderate edema. Electrocardiogram showed atrial fibrillation. Ultrasound: anterior mitral valve partial chordae tendineae rupture with prolapse, moderate regurgitation, large heart, apical four-chamber segmental mass in the right atrium, showing weak echo, swinging with the left and right of the cardiac cycle, no attachment to the free wall of the right atrium (Fig.)
