乳腺浸潤性導管癌超聲圖像特征與上皮—間葉轉化的關鍵因子Twist及E-cadherin表達的相關性研究

【摘要】:目的: 探討乳腺浸潤性導管癌(infiltrating ductal carcinoma, IDC)超聲聲像圖特征與上皮間葉轉化的關鍵性因子Twist、E-cadherin表達的相關性。旨在為浸潤型導管癌的惡性程度判定和預后評估提供一定的指導意義。 方法: 選擇經術后病理診斷證實為浸潤型導管癌的乳腺癌標本40例,所有患者術前未經過任何形式的放化療,入選病例術前均經彩色多普勒超聲檢查,采用免疫組織化學EnVision法檢測Twist、E-cadherin蛋白的表達情況。將其聲像圖特征(腫瘤大小、血流分級、腫物邊緣如毛刺征、微小鈣化、后方回聲衰減、縱橫比、同側腋窩淋巴結腫大及周圍軟組織侵犯等)與Twist、E-cadherin的表達情況進行比較,分析超聲影像特征與二者表達的相關性。 結果: 1.40例乳腺浸潤型導管癌中,超聲圖像腫瘤≥2cm31例,血流分級2-3級35例,邊緣不完整,有毛刺或小分葉者29例,縱橫比≥1者22例,同側腋窩淋巴結腫大25例,微小鈣化31例,周圍組織侵犯14例,后方回聲衰減16例。 2.40例乳腺浸潤型導管癌中,Twist蛋白陽性表達29例(72.5%),顯著高于癌旁正常組織(15.0%),差異有統(tǒng)計學意義(p0.05)。E-cadherin陽性表達22例(55.0%),顯著低于癌旁正常組織(100.0%),差異有統(tǒng)計學意義(p0.05)。 3. Twist在腫瘤邊緣不完整、同側腋窩淋巴結腫大、周圍軟組織侵犯中的陽性表達顯著高于邊緣完整、無同側腋窩淋巴結腫大及未侵犯周圍軟組織者(p0.05)。E-cadherin在腫瘤邊緣不完整、周圍軟組織侵犯中的陽性表達顯著低于邊緣完整及未侵犯周圍軟組織者(p0.05)。 結論: 1.乳腺浸潤性導管癌可表現為不同的超聲圖像及組織病理學特征。 2.浸潤性導管癌的超聲圖像與上皮間葉轉化的標志性因子Twist、E-cadherin的表達存在一定的關聯(lián)性,兩者結合在一定程度上可以判斷腫瘤侵襲、轉移等生物學行為,為浸潤性導管癌的惡性程度及預后判斷具有一定的指導意義。
[Abstract]:Objective: to investigate the correlation between (infiltrating ductal carcinoma, IDC) sonographic features and the expression of Twist,E-cadherin, a key factor of interepithelial lobe transformation in invasive ductal carcinoma of the breast. The purpose of this study is to provide guidance for the evaluation of malignant degree and prognosis of invasive ductal carcinoma. Methods: 40 cases of breast cancer with invasive ductal carcinoma confirmed by pathological diagnosis after operation were selected. All the patients did not undergo any form of radiotherapy and chemotherapy before operation. All the selected cases were examined by color Doppler ultrasound before operation. Immunohistochemical EnVision method was used to detect the expression of Twist,E-cadherin protein. The ultrasonographic features (tumor size, blood flow grade, margin of tumor such as burr sign, microcalcification, posterior echo attenuation, aspect ratio, ipsilateral axillary lymph node enlargement and surrounding soft tissue invasion, etc.) were compared with the expression of Twist,E-cadherin.
