【摘要】：目的:分析腹盆部孤立性纖維瘤(solitary fi brosis tumor,SFT)的CT表現(xiàn),提高診斷正確率。方法:分析2011年1月至2015年12月湘雅醫(yī)院經(jīng)病理證實(shí)的14例SFT的CT特征,并將10例腹盆部臟器外SFT與同時(shí)期10例腹盆部臟器外平滑肌肉瘤、28例神經(jīng)鞘瘤及18例胃腸外間質(zhì)瘤(extragastrointestinal stromal tumors,EGIST)的影像表現(xiàn)進(jìn)行對(duì)比分析。結(jié)果:腹盆部SFT大多呈"地圖樣"不均勻漸進(jìn)性強(qiáng)化,易液化壞死,瘤周常見(jiàn)迂曲血管。SFT分別與EGIST、神經(jīng)鞘瘤對(duì)比,其平掃CT值、動(dòng)脈期強(qiáng)化程度差異有統(tǒng)計(jì)學(xué)意義(P0.05);SFT分別與EGIST、平滑肌肉瘤對(duì)比,瘤周血管影出現(xiàn)率差異均有統(tǒng)計(jì)學(xué)意義(P0.008)。SFT與平滑肌肉瘤液化壞死率分類(lèi)差異有統(tǒng)計(jì)學(xué)意義(P0.008)。結(jié)論:腹盆部SFT的CT表現(xiàn)有一定特征,平掃CT值及動(dòng)脈期強(qiáng)化程度、壞死液化率、瘤周迂曲血管出現(xiàn)率對(duì)其與平滑肌肉瘤、神經(jīng)鞘瘤及EGIST的鑒別診斷有一定價(jià)值。
[Abstract]:Objective: to analyze the CT findings of (solitary fi brosis tumor in abdominal and pelvic solitary fibroma and to improve the diagnostic accuracy. Methods: Ct features of 14 cases of SFT confirmed by pathology in Xiangya Hospital from January 2011 to December 2015 were analyzed. The imaging findings of 10 cases of external SFT in abdominal and pelvic organs, 28 cases of neurilemmoma and 18 cases of (extragastrointestinal stromal tumors were compared with 10 cases of leiomyosarcoma of abdominal and pelvic organs in the same period. Results: most of SFT in abdominal and pelvic region showed "map" uneven and progressive enhancement, easy to liquefaction and necrosis. Compared with EGIST and neurilemmoma, SFT in peritumoral vessels were compared with plain CT value. Compared with EGIST and leiomyosarcoma, SFT had statistical significance (P0.008) SFT and leiomyosarcoma liquefaction necrosis rate (P0.008). Conclusion: the CT findings of abdominal and pelvic SFT have certain characteristics. The CT value of plain scan, the degree of enhancement in arterial phase, the rate of necrosis and liquefaction, and the occurrence rate of peri-tumor twisting vessels are valuable in the differential diagnosis of leiomyosarcoma, neurilemmoma and EGIST.
