應用二維及實時三維超聲心動圖評價正常人左房構型及功能的研究

【摘要】:第一部分應用左房追蹤技術探討正常人左房參數(shù)及影響因素 目的:應用左房追蹤技術探討正常人左房參數(shù)及其影響因素。 方法:選取202例正常人,其中男99例、女103例。男、女平均年齡分別為(41.5±12.5)歲、(44.9±13.1)歲,運用左房追蹤技術(left atrial tracking, LAT)獲得左房:最大上下徑(LALmax)、最小上下徑(LALmin)、最大周長(LACmax)、最小周長(LACmin)、最大面積(LAAmax)、最小面積(LAAmin)、最大容積(LAVmax)、最小容積(LAVmin)、收縮前容積(LApre)、收縮期充盈速率(dv/dtS)、左房舒張早期排空速率(dv/dtE)、左房舒張晚期排空速率(dv/dtA)參數(shù)的正常參考值范圍,并用體表面積(BSA)標化,研究其是否受性別、年齡因素影響。 結果:男性組與女性組比較,左房各參數(shù)均值差異無統(tǒng)計學意義(P㧐0.05)。左房各參數(shù)在不同年齡組之間差異具有統(tǒng)計學意義(P0.05),各參數(shù)隨年齡的增長而增加。 結論:左房追蹤技術測得正常人左房各參數(shù)隨著年齡的增長而增大,但與性別無關,作為新技術在探討正常人左房構型及功能方面具有一定的可行性。 第二部分應用實時三維超聲心動圖對正常人左房功能相關參數(shù)的研究 目的:應用實時三維超聲心動圖(real-time three-dimensionalechocardiography,RT-3DE)探討正常人左房容積及功能相關參數(shù)的正常值及其應用價值。 方法:選取202例健康志愿者,按性別分為男性組和女性組,按年齡分為A組≤40歲(63例),B組41~60歲(87例),C組㧐60歲(52例)。應用RT-3DE技術獲得左房最大容積(LAEDV)、左房最小容積(LAESV)、左房射血量(LASV)、左房收縮前容積(LApre)、左房被動排空容積(LAVp)、左房主動排空容積(LAVa)和左心房射血分數(shù)(LAEF),并對參數(shù)用體表面積校正,得出LAEDVI、LAESVI、LASVI、LApreI、LAVpI、LAVaI、LAEF’,統(tǒng)計分析得到上述參數(shù)正常值區(qū)間范圍,并比較各年齡組及性別組之間的差異。 結果:202例健康志愿者LAEDVI為(32.58~35.05)ml/m2,LAESVI為(14.80~16.21)ml/m2,LASVI為(17.87~19.40)ml/m2,LApreI為(17.99~19.86)ml/m2、LAVpI為(10.46~11.92)ml/m2、LAVaI為(6.88~7.92)ml/m2、LAEF’為(53.54~55.95)%。男性組與女性組比較LAEDVI、LAESVI、LASVI、LApreI、LAVpI、LAVaI、LAEF’值差異無統(tǒng)計學意義(P0.05)。不同年齡組間比較LAEDVI、LAESVI、LASVI、LApreI、LAVpI、LAVaI值差異有統(tǒng)計學意義(P0.05),LAEF’值在各年齡組間呈遞減趨勢,但差異無統(tǒng)計學意義(P0.05)。結論:左房各參數(shù)值與性別無關,,LAEDVI、LAESVI、LASVI、LApreI、LAVpI、LAVaI隨年齡增加而增大,LAEF’不隨年齡變化。RT-3DE技術具有實時、動態(tài)、立體、準確、無創(chuàng)等優(yōu)點,在評價左房容積及功能方面是可行的,為臨床能夠正確評估左房功能提供了新方法。
[Abstract]:The first part was to study the parameters of left atrium and its influencing factors in normal subjects by using left atrial tracing technique. Objective: to explore the parameters of left atrium and its influencing factors by using the technique of left atrial tracing. As a new technique, it is feasible to study the configuration and function of left atrium in normal subjects.
