【摘要】：平山病(Hirayama disease)是以單側(cè)上肢遠(yuǎn)端肌萎縮起病為特點(diǎn)的一種良性自限性疾病。早期因診斷技術(shù)限制及臨床認(rèn)識(shí)不足導(dǎo)致誤診率較高。近年來由于肌電圖和影像學(xué)檢查手段發(fā)展,臨床病例增多,平山病逐漸為人熟知。X線、CT可以清晰顯示患者頸椎曲度、局部節(jié)段穩(wěn)定性及椎間小關(guān)節(jié)退變程度、后縱韌帶是否骨化,而中立位頸椎MRI示失連接現(xiàn)象及屈頸位MRI發(fā)現(xiàn)硬膜前移則是最重要的影像學(xué)征象。本文將對(duì)平山病的臨床表現(xiàn)和肌電圖特點(diǎn)作一簡(jiǎn)要介紹,并對(duì)其影像學(xué)診斷技術(shù)及征象進(jìn)行詳細(xì)綜述。
[Abstract]:Pingshan disease (Hirayama disease) is a benign self-limited disease characterized by unilateral distal muscle atrophy. The misdiagnosis rate was higher in the early stage due to the limitation of diagnostic techniques and the lack of clinical knowledge. In recent years, due to the development of electromyography and imaging examination, the clinical cases of Pingshan disease are increasing. X-ray CT can clearly show the cervical curvature, the stability of local segment and the degree of degeneration of intervertebral facet joint. Whether the posterior longitudinal ligament is ossified or not, the most important imaging signs are the loss of connection on MRI in neutral cervical spine and the discovery of dural anterior movement in flexion cervical MRI. The clinical manifestations and electromyographic features of Pingshan disease are briefly introduced, and the imaging diagnostic techniques and signs are reviewed in detail.
