【摘要】：目的探討彩色多普勒超聲評估雷諾現(xiàn)象患者掌淺弓動脈、指掌側(cè)固有動脈病變的應(yīng)用價值。方法采用彩色多普勒超聲對病例組42例雷諾現(xiàn)象患者的左側(cè)掌淺弓動脈、左側(cè)中指指掌側(cè)固有動脈進行檢查,記錄血管走行、血流充盈情況及血流動力學(xué)參數(shù):收縮期峰值血流速度(PSV)、舒張末期血流速度(EDV)、阻力指數(shù)(RI)、搏動指數(shù)(PI)。對照組為35名接受體檢的正常健康成人,分別測量上述各參數(shù),比較病例組和對照組各參數(shù)的差異。結(jié)果病例組患者左側(cè)掌淺弓動血流充盈尚可,彩色血流顯示率為100%(42/42),左側(cè)中指指掌側(cè)固有動脈血流充盈差,血管變細、甚至閉塞,血管彩色血流示率為92.86%(39/42)。與對照組相比,病例組患者左側(cè)中指指掌側(cè)固有動脈PSV、EDV和左側(cè)掌淺弓動脈EDV減低,RI、PI均增高,比較差異均有統(tǒng)計學(xué)意義(P均0.05);病例組左側(cè)掌淺弓動脈PSV較對照組減低,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論彩色多普勒超聲能夠評估雷諾現(xiàn)象患者指掌側(cè)固有動脈、掌淺弓動脈血流動力學(xué)改變,可為雷諾現(xiàn)象的診斷、治療提供有效依據(jù)。
[Abstract]:Objective to evaluate the value of color Doppler ultrasound in evaluating the lesions of superficial palmar arch artery and digital palmar artery in patients with Renault phenomenon. Methods the left superficial palmar arch artery was examined by color Doppler ultrasound in 42 patients with Renault's phenomenon, and the palmar artery of the left middle finger was recorded. Filling and hemodynamic parameters: peak systolic velocity (PSV), end-diastolic velocity (EDV), resistance index (RI), pulsatile index (PI). The above parameters were measured in 35 healthy adults in the control group and the difference between the case group and the control group was compared. Results in the case group, the left superficial palmar arch was well filled, and the color blood flow was 100% (42 / 42). The blood flow of the palmar proper artery of the left middle finger became thin or even occluded. The color blood flow rate of the left middle finger was 92.86% (39 / 42). Compared with the control group, the EDV of the left medial digital palmar artery and the left superficial palmar arch increased significantly in the case group, and the PSV of the left superficial palmar arch was lower in the case group than that in the control group. The difference was not statistically significant (P0.05). Conclusion Color Doppler ultrasound can evaluate the hemodynamic changes of the proper digital palmar artery and superficial palmar arch in patients with Raynaud's phenomenon, which can provide an effective basis for the diagnosis and treatment of Raynaud's phenomenon.
