【摘要】：目的探討超聲造影(CEUS)鑒別診斷乳腺良惡性腫瘤的應(yīng)用價(jià)值。方法納入277例乳腺腫瘤患者,均行常規(guī)超聲(BUS)及CEUS檢查,并與病理結(jié)果進(jìn)行對(duì)照,對(duì)BUS及CEUS各觀察指標(biāo)進(jìn)行Logistic回歸分析,比較BUS與CEUS鑒別診斷乳腺良惡性腫瘤的效能。結(jié)果 277例患者中,良性病變184例(良性組),惡性病變93例(惡性組)。Logistic回歸分析顯示,BUS診斷惡性腫瘤的危險(xiǎn)指標(biāo)為中等血供、非平行生長(zhǎng)、血供豐富及邊界模糊,相對(duì)危險(xiǎn)度分別為5.17、4.84、3.39、3.04;CEUS為離心性增強(qiáng)、向心性增強(qiáng)、腫瘤形狀不規(guī)則、造影劑分布不均、增強(qiáng)后范圍增大,相對(duì)危險(xiǎn)度分別為13.36、4.58、4.07、3.28、2.36。BUS診斷乳腺腫瘤的準(zhǔn)確率為81.59%(226/277),CEUS為88.09%(244/277),二者差異有統(tǒng)計(jì)學(xué)意義(χ2=1.012,P0.001)。結(jié)論采用CEUS鑒別診斷乳腺良惡性腫瘤的準(zhǔn)確率較高,其在乳腺局灶性病變的定性診斷中具有一定的應(yīng)用價(jià)值。
[Abstract]:Objective to evaluate the value of contrast-enhanced (CEUS) in differential diagnosis of breast benign and malignant tumors. Methods (BUS) and CEUS were performed in 277 patients with breast neoplasms, and compared with pathological results, Logistic regression analysis was performed on BUS and CEUS. The effectiveness of BUS and CEUS in differential diagnosis of benign and malignant breast tumors was compared. Results among 277 patients, 184 cases were benign lesions (benign group), 93 cases were malignant lesions (malignant group) .Logistic regression analysis showed that the risk index for diagnosis of malignant tumor was medium blood supply, non-parallel growth, abundant blood supply and fuzzy boundary. The relative risk was 5.17 ~ 4.84 ~ 3.39 ~ 3.04 CEUs for centrifugal enhancement, concentric enhancement, irregular shape of tumor, uneven distribution of contrast media, and increased range after enhancement. The relative risk was 13.36 / 4.587 / 3.28 / 2.36. Bus was 81.59% (226 / 277) and CEUS was 88.09% (244 / 277), respectively. The difference was statistically significant (蠂 ~ 2 ~ (2) n ~ (1) 12) (P ~ (0.001). Conclusion the accuracy of CEUS in differential diagnosis of breast benign and malignant tumors is high, and it has certain application value in qualitative diagnosis of local breast lesions.
