【摘要】：目的應(yīng)用動(dòng)脈分析技術(shù)分析妊娠期高血壓(PIH)患者的頸動(dòng)脈血管彈性及徑向應(yīng)變、應(yīng)變率,并與妊娠期無(wú)高血壓的正常孕婦進(jìn)行對(duì)比研究。方法選取于我院就診的PIH患者28例作為病例組(PIH組),同期選取正常妊娠女性30例作為對(duì)照組(正常妊娠組)。應(yīng)用動(dòng)脈分析技術(shù)通過(guò)追蹤頸動(dòng)脈血管壁的運(yùn)動(dòng),自動(dòng)獲取頸動(dòng)脈彈性參數(shù)及徑向應(yīng)變、應(yīng)變率等數(shù)據(jù),并進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果 PIH組的頸動(dòng)脈血管僵硬度β、脈搏波傳導(dǎo)速度較正常妊娠組明顯增高,而血管順應(yīng)性則較正常妊娠組明顯減低,差異均有統(tǒng)計(jì)學(xué)意義(P0.05),反射波增強(qiáng)指數(shù)的差異無(wú)統(tǒng)計(jì)學(xué)意義。PIH組整體徑向應(yīng)變、整體徑向應(yīng)變率較正常妊娠組相比明顯減低,差異有統(tǒng)計(jì)學(xué)意義(P0.05),其中徑向應(yīng)變與脈搏波傳導(dǎo)速度呈顯著負(fù)相關(guān)。結(jié)論血管整體徑向應(yīng)變和應(yīng)變率可以作為反映PIH患者血管彈性變化的有效指標(biāo),且徑向應(yīng)變比徑向應(yīng)變率更加敏感。動(dòng)脈分析技術(shù)通過(guò)以上反映頸動(dòng)脈功能的敏感指標(biāo),可以及早發(fā)現(xiàn)PIH患者血管彈性的減低,在血管形態(tài)學(xué)未發(fā)生改變之前為臨床預(yù)防和治療提供依據(jù)。
[Abstract]:Objective to analyze the carotid artery elasticity, radial strain and strain rate in patients with hypertension complicating pregnancy (PIH) by arterial analysis, and to compare the results with those of normal pregnant women without hypertension during pregnancy. Methods 28 PIH patients in our hospital were selected as the case group (PIH group) and 30 normal pregnant women as the control group (normal pregnancy group). By tracking the movement of carotid artery wall, the data of carotid elastic parameters, radial strain and strain rate were obtained automatically by means of arterial analysis, and statistical analysis was carried out. Results the carotid artery stiffness 尾 and pulse wave velocity in PIH group were significantly higher than those in normal pregnancy group, while vascular compliance was significantly lower in PIH group than in normal pregnancy group. All the differences were statistically significant (P0.05). There was no significant difference in reflection wave enhancement index. The global radial strain rate of PIH group was significantly lower than that of normal pregnancy group. The difference was statistically significant (P0.05), in which radial strain and pulse wave velocity were negatively correlated. Conclusion Radial strain and strain rate can be used as an effective index to reflect vascular elasticity in PIH patients, and radial strain is more sensitive than radial strain rate. Through the above sensitive indexes which reflect carotid artery function, the reduction of vascular elasticity in PIH patients can be detected as early as possible, which can provide evidence for clinical prevention and treatment before the change of vascular morphology.
