【摘要】：目的探討超聲造影在肝細(xì)胞癌(HCC)和肝局灶性結(jié)節(jié)增生(FNH)鑒別診斷中的應(yīng)用價值。方法收集該院腫瘤科2012年6月至2015年1月收治的35例HCC患者和30例FNH患者,分別應(yīng)用超聲檢查,比較兩組超聲檢查定量參數(shù)和評價超聲鑒別診斷價值。結(jié)果超聲檢查HCC的注入造影劑到最大峰值強度增高1/2時間(RT)短于FNH的超聲結(jié)果,從開始增強到造影劑聚集最大時所需時間(TTP)、造影劑平均通過時間(MTT)、峰值強度(PI)均長于FNH的超聲結(jié)果(均P0.05);超聲診斷HCC靈敏度、特異度、一致率、陽性預(yù)測值、陰性預(yù)測值分別為94.29%、83.33%、89.23%、86.84%、92.59%,對FNH分別為83.33%、94.29%、89.23%、92.59%、86.84%,差異均無統(tǒng)計學(xué)意義(P0.05)。結(jié)論超聲造影在HCC和FNH鑒別診斷中具有重要臨床價值,均具有較高靈敏度和特異度,值得臨床推薦。
[Abstract]:Objective to evaluate the value of contrast-enhanced ultrasonography in differential diagnosis of hepatocellular carcinoma (HCC) and focal nodular hyperplasia (FNH). Methods from June 2012 to January 2015, 35 patients with HCC and 30 patients with FNH were examined by ultrasound. The quantitative parameters of ultrasound examination and the value of ultrasonic differential diagnosis were compared between the two groups. Results Ultrasound examination of HCC showed that the maximum peak intensity increased by 1 / 2 time (RT) less than that of FNH. The mean transit time (MTT) and peak intensity (Pi) of contrast media were longer than those of FNH (P0.05), the sensitivity, specificity, consistency rate, positive predictive value of HCC were higher than those of FNH (P 0.05), while the mean transit time (MTT) and peak intensity (Pi) of contrast agent were higher than that of FNH (P0.05), the sensitivity, specificity, consistency rate and positive predictive value of HCC were higher than those of FNH. The negative predictive value was 94.290.83.33 and 86.842.59, respectively, and the FNH was 83.330.94.29and 89.232.599.The difference was not statistically significant (P0.05). Conclusion Contrast-enhanced ultrasonography has important clinical value in the differential diagnosis of HCC and FNH, and has high sensitivity and specificity, which is worthy of clinical recommendation.
