【摘要】：目的探討胃癌累及或突破小網(wǎng)膜的CT增強(qiáng)表現(xiàn),以期為臨床治療提供可靠依據(jù)。方法回顧性研究重慶市江北區(qū)中醫(yī)院2013年7月至2014年6月經(jīng)外科證實(shí)病變累及或突破小網(wǎng)膜,同時(shí)具有術(shù)前上腹部CT增強(qiáng)檢查資料的胃癌患者61例。依據(jù)外科手術(shù)發(fā)現(xiàn)對(duì)照研究相關(guān)CT結(jié)果,觀察胃癌病灶的胃癌病灶的形態(tài)、部位,CT在小網(wǎng)膜受累中的征象,胃癌病變向外擴(kuò)散滿意的特點(diǎn)及腹水伴隨情況。結(jié)果術(shù)前CT增強(qiáng)顯示小網(wǎng)膜受胃癌直接浸潤34.43%(21/61),種植轉(zhuǎn)移29.51%(18/61),小網(wǎng)膜囊鄰近淋巴結(jié)轉(zhuǎn)移63.93%(39/61);伴不同程度腹腔積液16例;有無腹水與胃癌直接對(duì)小網(wǎng)膜侵犯、胃癌小網(wǎng)膜淋巴結(jié)轉(zhuǎn)移差異無統(tǒng)計(jì)學(xué)意義(P0.05),與胃癌小網(wǎng)膜種植轉(zhuǎn)移差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論胃癌術(shù)前上腹部CT增強(qiáng)檢查可較準(zhǔn)確評(píng)價(jià)小網(wǎng)膜受累狀況,由此可指導(dǎo)臨床治療方案的制訂及評(píng)估預(yù)后。
[Abstract]:Objective to investigate the CT enhancement features of gastric cancer involving or breaking through the omentum in order to provide reliable evidence for clinical treatment. Methods from July 2013 to June 2014, 61 patients with gastric cancer who were confirmed by surgery to break through the lesser omentum and had the data of CT enhancement before operation were retrospectively studied in Jiangbei traditional Chinese Medicine Hospital of Chongqing. According to the CT findings of surgical findings, the morphology of gastric cancer focus, the CT features of location CT in the involvement of lesser omentum, the characteristics of the diffusion of gastric cancer to the outside and the concomitant condition of ascites were observed. Results preoperative CT enhancement showed that the omentum was directly infiltrated by gastric carcinoma 34.43% of 61%, the implantation metastasis was 29.51%, and the adjacent lymph node metastasis of the omentum sac was 63.93% of 63.93%. There were 16 cases of peritoneal effusion with different degrees of ascites and gastric cancer directly invading the lesser omentum. There was no significant difference in lymph node metastasis between the small omentum of gastric cancer and that of the implantation of omentum in gastric carcinoma (P 0.05). Conclusion preoperative upper abdominal CT enhanced examination can accurately evaluate the situation of omentum involvement, which can be used to guide the clinical treatment and evaluate the prognosis.
