MRI乳腺影像報(bào)告和數(shù)據(jù)系統(tǒng)在非腫塊強(qiáng)化病變中的診斷價(jià)值研究

【摘要】:目的探討MRI乳腺影像報(bào)告和數(shù)據(jù)系統(tǒng)(BI-RADS)在非腫塊強(qiáng)化病變的診斷和鑒別診斷價(jià)值。方法搜集2010年10月至2013年1月接受MRI檢查并經(jīng)病理證實(shí)或臨床隨診1年以上的203例非腫塊強(qiáng)化患者資料,根據(jù)BI-RADS分類研究其動(dòng)態(tài)增強(qiáng)形態(tài)學(xué)特征和時(shí)間-信號(hào)強(qiáng)度曲線(TIC)、進(jìn)行分類并與病理學(xué)結(jié)果相對(duì)照。結(jié)果 203例患者中共209個(gè)非腫塊強(qiáng)化病灶,良性病變58個(gè)(27.75%),惡性病變151個(gè)(72.25%)。良性病變中以腺病最多,占25.86%(15/58);惡性病變中浸潤(rùn)性導(dǎo)管癌最多,占47.68%(72/151),且均伴有導(dǎo)管原位癌成分。在形態(tài)學(xué)上,良惡性病變?cè)诰衷钚苑植、?jié)段性分布、簇狀環(huán)形強(qiáng)化間的差異有統(tǒng)計(jì)學(xué)意義(P值分別為0.001、0.003、0.010);良惡性病變?cè)赥IC類型Ⅰ型和Ⅲ型的差異有統(tǒng)計(jì)學(xué)意義(P值分別為0.000、0.018)。惡性病變的危險(xiǎn)性相關(guān)因素包括節(jié)段性分布、簇狀環(huán)形強(qiáng)化、TIC為Ⅲ型(OR值分別為3.5、4.22、2.48)。MRI BI-RADS分類對(duì)非腫塊惡性病變?cè)\斷的敏感性、特異性、準(zhǔn)確性分別為94.04%、55.17%、83.25%。結(jié)論 MRI BI-RADS是診斷和鑒別診斷乳腺非腫塊強(qiáng)化病變的有效方法,具有較高的敏感性和準(zhǔn)確性,但特異性較低。
[Abstract]:Objective to evaluate the value of MRI imaging reporting and data system (BI-RADS) in the diagnosis and differential diagnosis of non-mass enhanced lesions. Methods from October 2010 to January 2013, 203 patients with non-mass enhancement underwent MRI examination and were confirmed by pathology or followed up for more than one year. According to the classification of BI-RADS, the dynamic enhanced morphological characteristics and time-signal intensity curve were studied, and the results were compared with the pathological results. Results in 203 patients, 209 lesions were non-mass enhanced, 58 benign lesions were involved in 27.755.151 malignant lesions were found in 72.25 cases. Among benign lesions, adenopathy was the most, accounting for 25.86%, and invasive ductal carcinoma was the most, accounting for 47.68% of carcinoma in situ.
