【摘要】：目的探尋右心房黏液瘤的超聲特點(diǎn),評(píng)價(jià)超聲心動(dòng)圖對(duì)右心房黏液瘤的診斷價(jià)值。方法回顧分析20例右心房黏液瘤的超聲表現(xiàn),總結(jié)右心房黏液瘤的超聲心動(dòng)圖特點(diǎn)。結(jié)果右心房黏液瘤的臨床癥狀主要有胸悶、氣短、下肢浮腫及暈厥等。超聲測(cè)量瘤體大小為3.11 cm×2.90 cm~7.44 cm×4.52 cm,形態(tài)為類圓形或分葉狀。瘤蒂附著于房間隔者占75%,附著于右心房游離壁者占25%。瘤蒂平均寬度為(1.62±0.30)cm,寬度大于1 cm者占80%。部分黏液瘤內(nèi)部可見鈣化和囊狀無(wú)回聲區(qū)。右心房?jī)?nèi)血流阻塞占70%,三尖瓣狹窄占50%,肺高壓者占10%。結(jié)論超聲心動(dòng)圖可以準(zhǔn)確評(píng)價(jià)右心房黏液瘤瘤體特點(diǎn)及繼發(fā)心內(nèi)結(jié)構(gòu)和血流動(dòng)力學(xué)改變,為臨床作出及時(shí)、準(zhǔn)確診斷提供重要依據(jù)。
[Abstract]:Objective to explore the ultrasonic characteristics of right atrial myxoma and evaluate the diagnostic value of echocardiography for right atrial myxoma. Methods the ultrasonic features of 20 cases of right atrial myxoma were reviewed and analyzed, and the echocardiographic characteristics of right atrial myxoma were summarized. The main clinical symptoms of right atrial myxoma were chest tightness, shortness of breath, swelling of lower limbs and syncope. The size of the tumor was 3.11 cm x 2.90 cm~7.44 cm x 4.52 cm, and the morphology was round or lobulated. The tumor pedicle attached to the atrial septum accounted for 75%. The average width of the 25%. pedicle attached to the right atrium was (1.62 + 0.30) cm, and the width was greater than 1 cm in the inner part of the 80%. part of the myxoma. The blood flow in the right atrium was visible. Obstruction accounted for 70%, three apical stenosis accounted for 50%. 10%. conclusion echocardiography in patients with pulmonary hypertension could accurately evaluate the characteristics of right atrial myxoma and secondary cardiac structure and hemodynamic changes, providing an important basis for timely and accurate diagnosis.
