乳腺癌患者癌灶動(dòng)態(tài)增強(qiáng)磁共振成像表現(xiàn)與ER、PR、Akt、p27表達(dá)的相關(guān)性

【摘要】:目的探討乳腺癌動(dòng)態(tài)增強(qiáng)磁共振成像(DCE-MRI)表現(xiàn)與ER、PR、Akt、p27表達(dá)的相關(guān)性,探討用乳腺癌DCE-MRI表現(xiàn)預(yù)測(cè)乳腺癌ER、PR、Akt和p27表達(dá)的可能性。方法回顧150例乳腺癌患者156個(gè)病灶的DCE-MRI表現(xiàn),包括病灶最大徑、病灶形狀、病灶邊緣情況、DCE-MRI強(qiáng)化形式、時(shí)間信號(hào)強(qiáng)度曲線(TIC)類型、早期增強(qiáng)率。采用免疫組化Elivision法檢測(cè)156個(gè)乳腺癌病灶的ER、PR、Akt、p27表達(dá)情況。將病灶按DCE-MRI病灶最大徑、病灶形狀、病灶邊緣情況、DCE-MRI強(qiáng)化形式、TIC類型、早期增強(qiáng)率分為不同亞組,采用χ~2檢驗(yàn)比較各亞組ER、PR、Akt、p27陽(yáng)性表達(dá)率。采用Spearman相關(guān)性分析法計(jì)算病灶最大徑、病灶形狀、病灶邊緣情況、DCEMRI強(qiáng)化形式、TIC類型、早期強(qiáng)化率與ER、PR、Akt和p27表達(dá)的相關(guān)系數(shù)(r)和P值。結(jié)果本組乳腺癌病灶最大徑≤2 cm者84個(gè)、2 cm者72個(gè);病灶形態(tài)類圓形者23個(gè)、分葉形51個(gè)、不規(guī)則形62個(gè);病灶邊緣光滑9個(gè)、不規(guī)則62個(gè)、毛刺85個(gè);DCE-MRI強(qiáng)化形式為均勻強(qiáng)化者31個(gè)、不均勻強(qiáng)化者80個(gè)、環(huán)形強(qiáng)化者45個(gè);TIC類型Ⅰ型10個(gè)、Ⅱ型59個(gè)、Ⅲ型87個(gè);早期強(qiáng)化率50%者11個(gè)、50%~100%者66個(gè)、100%者79個(gè)。156個(gè)乳腺癌病灶中ER表達(dá)陽(yáng)性122個(gè)(78.21%),PR表達(dá)陽(yáng)性122個(gè)(78.21%),Akt表達(dá)陽(yáng)性104個(gè)(66.67%)、p27陽(yáng)性106個(gè)(67.95%)。腫病灶邊緣情況與ER表達(dá)有關(guān)(χ~2=4.662,P0.05)。病灶邊緣情況、DCE-MRI強(qiáng)化形式與PR表達(dá)有關(guān)(χ~2=4.515、6.017,P均0.05)。早期增強(qiáng)率與Akt表達(dá)有關(guān)(χ~2=6.211,P0.05)。病灶最大徑、病灶形態(tài)、TIC類型與p27表達(dá)有關(guān)(χ~2=5.932、4.851、5.828,P均0.05)。Spearman相關(guān)性分析結(jié)果顯示,病灶邊緣情況與ER陽(yáng)性表達(dá)有關(guān)(r為0.594,P0.05),病灶邊緣情況、DCE-MRI強(qiáng)化形式與PR陽(yáng)性表達(dá)有關(guān)(r分別為6.13、-0.558,P均0.05),早期增強(qiáng)率與Akt陽(yáng)性表達(dá)有關(guān)(r為0.587,P0.05),乳腺癌病灶最大徑、病灶形態(tài)、TIC類型與p27陽(yáng)性表達(dá)有關(guān)(r分別為0.721、0.689、0.707,P均0.05)。結(jié)論乳腺癌病灶邊緣情況與ER陽(yáng)性表達(dá)有關(guān),病灶邊緣情況、DCE-MRI強(qiáng)化形式與PR陽(yáng)性表達(dá)有關(guān),早期增強(qiáng)與Akt陽(yáng)性表達(dá)有關(guān),乳腺癌病灶最大徑、病灶形態(tài)、TIC類型與p27陽(yáng)性表達(dá)有關(guān)。
[Abstract]:Objective to investigate the correlation between dynamic enhanced magnetic resonance imaging (DCE-MRI) of breast cancer and the expression of Aktp27 in ERT PRA, and to explore the possibility of predicting the expression of Akt and p27 by DCE-MRI in breast cancer. Methods the DCE-MRI findings of 156 lesions in 150 patients with breast cancer were retrospectively reviewed, including the maximum diameter of the lesion, the shape of the lesion, the enhanced form of DCE-MRI, the time signal intensity curve and the early enhancement rate. Immunohistochemical Elivision method was used to detect the expression of ERP P 27 in 156 breast cancer lesions. According to the maximum diameter, shape and edge of DCE-MRI, the early enhancement rate was divided into different subgroups, and the positive expression rate of ERP PRA AktP27 in each subgroup was compared by 蠂 ~ 2 test. Spearman correlation analysis was used to calculate the maximum diameter, shape and edge of the lesion. The correlation coefficient of early enhancement rate with the expression of Akt and p27 in ERV PRT and p27 was calculated. Results in this group, the largest diameter of breast cancer was 84 or 2 cm in diameter, 23 lesions were round in shape, 51 in lobular shape, 62 in irregular shape, 9 in smooth margin and 62 in irregular shape. DCE-MRI of 85 burrs showed homogeneous enhancement in 31 cases, uneven enhancement in 80 cases, circular enhancement in 45 cases in type 鈪,
