【摘要】：目的:本研究通過建立正中神經(jīng)在腕管入口與出口處橫截面積的比值,闡明其對腕管綜合征的診斷價值。方法:46個經(jīng)臨床檢查和電生理檢測確診的腕管綜合征患者和44個健康志愿者均進行腕部超聲檢測。分別在腕管入口(舟骨-豌豆骨水平)和腕管出口(鉤骨鉤水平)處測量正中神經(jīng)的橫截面積,并計算相應(yīng)的入口與出口處橫截面積的比值。超聲檢測和電生理檢查在雙盲條件下進行。電生理檢測結(jié)合相應(yīng)的臨床癥狀被認為是診斷腕管綜合征的金標準。用ROC曲線來評價此比值的診斷準確性。結(jié)果:實驗組:16男,30女(平均45.3歲,18-83歲)。對照組:18男,26女(平均50.4歲,18-79歲)。正中神經(jīng)腕管入口處的橫截面積實驗組和對照組分別為為14.6 mm2和8.7 mm2(P0.001)。對照組面積比為1.0,小于實驗組中的面積比1.6(P0.001)。ROC曲線分析表明面積比的診斷價值優(yōu)于入口處橫截面積(P0.001)。當面積比取值為1.3時,診斷的特異度和敏感度分別為93%和91%。結(jié)論:正中神經(jīng)在腕管入口與出口處橫截面積的比值是一種診斷腕管綜合征的有效方法,但是需要大規(guī)模的隨機對照試驗進一步研究。
[Abstract]:Objective: to clarify the diagnostic value of the median nerve at the transverse area of the carpal tunnel entrance and the outlet of the carpal tunnel. Methods: 46 patients with carpal tunnel syndrome and 44 healthy volunteers were examined by clinical examination and electrophysiological examination. The transverse area of the median nerve was measured at the exit of the carpal canal (the hook level of the carpal hook) and the ratio of the corresponding cross section of the entrance to the exit was calculated. The ultrasonic detection and electrophysiological examination were carried out under double blindness. The electrophysiological tests combined with the corresponding clinical symptoms were considered as the gold standard for the diagnosis of carpal tunnel syndrome. The ROC curve was used to evaluate this. The diagnostic accuracy of the ratio. Results: the experimental group: 16 men and 30 women (average 45.3 years, 18-83 years old). The control group was 18 men and 26 women (average 50.4 years, 18-79 years old). The experimental group and the control group were 14.6 mm2 and 8.7 mm2 (P0.001) at the median nerve carpal tunnel entrance. The area ratio of the group was 1, and the area ratio of the experimental group was compared to 1.6 (P0.001).ROC koji. Line analysis shows that the diagnostic value of area ratio is better than that of the transversal area of the entrance (P0.001). When the area ratio is 1.3, the specificity and sensitivity of the diagnosis are 93% and 91%., respectively. The ratio of the median nerve at the entrance of the carpal tunnel and the exit is an effective method for the diagnosis of carpal tunnel syndrome, but it requires a large random pair. Further study was carried out according to the test.
