【摘要】：目的：探討常規(guī)超聲對(duì)原發(fā)性膽囊癌的診斷及鑒別診斷價(jià)值。 方法：對(duì)318例膽囊病變進(jìn)行常規(guī)超聲檢查,觀察超聲征象特征,將診斷結(jié)果與病理進(jìn)行比較。重點(diǎn)分析灰階超聲征象單項(xiàng)指標(biāo)的診斷作用、鑒別診斷方法及誤診原因。 結(jié)果：(1)灰階超聲單項(xiàng)診斷指標(biāo)包括：①病灶形態(tài)不規(guī)則或膽囊壁毛糙；②病灶寬基底或膽囊壁局限性增厚；③病變處囊壁連續(xù)性中斷；④單發(fā)病灶；⑤病灶直徑≥10mm或膽囊壁厚度3mm；⑥伴發(fā)膽囊結(jié)石。比較膽囊良惡性病變差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。(2)與病理比較,常規(guī)超聲診斷原發(fā)性膽囊癌的靈敏度、特異度、假陽(yáng)性率、假陰性率、陽(yáng)性預(yù)測(cè)值、陰性預(yù)測(cè)值分別為73.3%、97.2%、26.7%、2.8%、73.3%、97.2%。 結(jié)論：常規(guī)超聲對(duì)原發(fā)性膽囊癌的診斷及鑒別診斷有重要價(jià)值�；译A超聲征象單項(xiàng)指標(biāo)對(duì)膽囊良惡性病變有診斷指導(dǎo)意義。
[Abstract]:Objective: to evaluate the value of conventional ultrasound in the diagnosis and differential diagnosis of primary gallbladder carcinoma. Methods: 318 cases of cholecystic diseases were examined by conventional ultrasound, and the features of ultrasonic signs were observed. The diagnostic results were compared with pathology. The diagnostic function, differential diagnosis method and misdiagnosis reason of single index of gray-scale ultrasonic sign were analyzed. Results the single diagnostic index of gray-scale ultrasound included the irregular shape of the lesions in 1 / 1 or the width of the base of the lesions or the local thickening of the gallbladder wall (3) the continuity of the cystic wall in the lesions was interrupted and 4 single lesions were detected.
