【摘要】：目的探討肺原發(fā)性黏液表皮樣癌(MEC)的多層螺旋CT(MSCT)表現(xiàn)。方法回顧性分析經(jīng)病理證實(shí)的24例肺原發(fā)性MEC的MSCT資料,所有患者術(shù)前均行CT平掃及增強(qiáng)掃描。結(jié)果本組24例均表現(xiàn)為單發(fā)病灶。CT平掃顯示中央型17例,其中腔內(nèi)息肉型14例,合并阻塞性肺炎及肺不張11例;腔內(nèi)外腫塊型2例,合并阻塞性肺炎及肺氣腫1例,合并肺泡積血1例;管腔全層浸潤(rùn)型1例,增厚的管壁可見散在鈣化,氣道輕度狹窄。外周型7例,3例見分葉及毛刺,3例見不規(guī)則厚壁空洞。增強(qiáng)后輕度強(qiáng)化4例,中度強(qiáng)化11例,明顯強(qiáng)化9例。3例合并肺門及縱隔淋巴結(jié)轉(zhuǎn)移,其中1例合并骨轉(zhuǎn)移。結(jié)論中央型肺MEC好發(fā)于年輕人,多表現(xiàn)為邊緣光滑的圓形或類圓形腔內(nèi)腫塊,增強(qiáng)后中度或明顯強(qiáng)化;而外周型MEC好發(fā)于中老年人,缺乏特征性CT表現(xiàn),確診最終依賴病理。
[Abstract]:Objective to investigate the multi-slice spiral CT (MSCT) manifestations of primary pulmonary mucoepidermoid carcinoma (MEC). Methods the MSCT data of 24 cases of primary pulmonary MEC confirmed by pathology were retrospectively analyzed. All the patients underwent CT scan and enhanced scan before operation. Results in this group, 24 cases showed 17 cases of central type with single focal.CT scan, including 14 cases of polypoid type in the cavity. There were 11 cases of obstructive pneumonia and atelectasis, 2 cases of lumen mass, 1 cases of obstructive pneumonia and emphysema, 1 cases of pulmonary alveolus combined with alveolus, 1 cases of full layer infiltration in the lumen. The thickened wall of the tube could be seen scattered in calcification, mild airway stenosis, 7 cases of peripheral type, 3 cases of lobules and burrs, and 3 cases with irregular thick wall cavity. 4 cases were slightly enhanced after enhancement, 4 and moderate. Intensification of 11 cases, obviously enhanced 9 cases of.3 cases with hilar and mediastinal lymph node metastasis, of which 1 cases were combined with bone metastasis. Conclusion central lung MEC is well distributed in young people, most of them are round or round lumen lumps with smooth edges, and after enhanced medium or obvious enhancement, but peripheral MEC is in the middle and old age, lacks characteristic CT performance and final diagnosis is final.
