【摘要】：目的探討多脾綜合征患者合并心臟畸形的特點(diǎn),為超聲診斷及外科治療提供思路。方法采用超聲心動(dòng)圖顯示45例多脾綜合征患者心臟畸形并分析其特點(diǎn),回顧分析34例多脾綜合征患者外科手術(shù)方式特點(diǎn)。結(jié)果 45例多脾綜合征患者均存在房間隔發(fā)育異常;40例(88.9%)合并室問隔發(fā)育異常;房室瓣發(fā)育異常發(fā)生率較高,共25例(55.6%);21例(46.7%)患者存在動(dòng)脈-心室連接異常;32例(71.1%)患者存在右室流出道及肺動(dòng)脈發(fā)育異常;39例(86.7%)存在體靜脈及肺靜脈引流異常。34例患者接受外科手術(shù),包括雙心室矯治及Fontan類手術(shù)。結(jié)論多脾綜合征多合并復(fù)雜心臟畸形,超聲心動(dòng)圖可以較準(zhǔn)確診斷多脾綜合征及其心臟畸形情況,決策外科手術(shù)方式。
[Abstract]:Objective to investigate the characteristics of heart malformation in patients with polysplenic syndrome and to provide ideas for ultrasonic diagnosis and surgical treatment. Methods 45 patients with polysplenic syndrome were examined by echocardiography and their characteristics were analyzed. The surgical methods of 34 patients with polysplenic syndrome were analyzed retrospectively. Results Atrial septal dysplasia was found in all 45 patients with polysplenic syndrome (n = 40) accompanied with abnormal development of interventricular septum, and the incidence of abnormal development of atrioventricular valve was higher. There were 32 cases with abnormal arterial-ventricular connection (32 cases with abnormal arterial-ventricular connection) and 39 cases with abnormal development of right ventricular outflow tract and pulmonary artery (39 cases with abnormal development of right ventricular outflow tract and pulmonary artery). 34 cases with abnormal drainage of systemic vein and pulmonary vein were operated on. Including biventricular correction and Fontan type surgery. Conclusion Polysplenic syndrome is complicated with complex cardiac malformation. Echocardiography can accurately diagnose polysplenic syndrome and its cardiac malformation, and make surgical decision.
