聲觸診定量技術(shù)與聲觸診成像定量技術(shù)對(duì)甲狀腺結(jié)節(jié)性質(zhì)的判斷價(jià)值比較

【摘要】:目的比較聲觸診定量技術(shù)(VTQ)與聲觸診成像定量技術(shù)(VTIQ)在鑒別甲狀腺結(jié)節(jié)性質(zhì)中的應(yīng)用價(jià)值。方法選擇甲狀腺結(jié)節(jié)患者100例(118枚結(jié)節(jié)),經(jīng)手術(shù)或結(jié)節(jié)穿刺病理證實(shí)良性結(jié)節(jié)41枚、惡性結(jié)節(jié)77枚。采用Siemens Acuson S3000型超聲診斷儀分別于VTQ及VTIQ模式下測量剪切波速度(SWV);繪制VTQ與VTIQ對(duì)結(jié)節(jié)性質(zhì)判斷的受試者工作特性曲線,計(jì)算曲線下面積(AUC),計(jì)算兩種方法對(duì)結(jié)節(jié)性質(zhì)的判斷效能。結(jié)果VTQ測量失敗率為5%(6/118),良、惡性結(jié)節(jié)的SWV分別為(2.43±0.52)、(3.68±0.91)m/s,二者比較P0.01。VTIQ測量失敗率為0,良、惡性結(jié)節(jié)的SWV分別為(2.34±0.85)、(3.81±1.07)m/s,二者比較P0.01。VTQ與VTIQ的AUC分別為0.621(95%CI:0.512~0.730)、0.723(95%CI:0.621~0.825)。VTIQ判斷結(jié)節(jié)性質(zhì)的敏感性、特異性、陽性預(yù)測值、陰性預(yù)測值及準(zhǔn)確率均高于VTQ。VTQ與VTIQ判斷直徑≤1 cm結(jié)節(jié)性質(zhì)的敏感性、特異性、陽性預(yù)測值、陰性預(yù)測值及準(zhǔn)確率均高于直徑1 cm結(jié)節(jié),且VTIQ均高于VTQ。結(jié)論 VTQ與VTIQ對(duì)于直徑≤1 cm甲狀腺結(jié)節(jié)性質(zhì)的判斷效能均較高,但VTIQ的總體判斷效能高于VTQ。
[Abstract]:Objective to compare the value of acoustic palpation quantitative technique (VTQ) and acoustic palpation imaging quantitative technique (VTIQ) in distinguishing thyroid nodule. Methods one hundred patients with thyroid nodules were selected, including 41 benign nodules and 77 malignant nodules confirmed by operation or nodule puncture pathology. The shear wave velocities were measured by Siemens Acuson S3000 ultrasonic diagnostic instrument in VTQ and VTIQ mode respectively, and the operating characteristic curves of VTQ and VTIQ to judge the nodular properties were plotted, the area under the curve was calculated, and the effectiveness of two methods to judge the nodular properties was calculated. Results the failure rate of VTQ was 5 / 6 / 118, the SWV of benign and malignant nodules was 2. 43 鹵0. 52 鹵0. 91 m / s, the SWV of benign and malignant nodules was 0, and the SWV of benign and malignant nodules was 2. 34 鹵0. 85 鹵1. 07 m / s, respectively. The AUC of P0.01.VTQ and VTIQ were 0. 621%, 0. 75%, 0. 730% and 0. 723 95% respectively.
