【摘要】：目的:探討超聲測(cè)量肝前脂肪厚度對(duì)診斷代謝綜合征的價(jià)值。方法 :對(duì)496例內(nèi)分泌科隨訪的社區(qū)人群按代謝綜合征相關(guān)各組分疾病分組,用超聲測(cè)量肝前脂肪厚度、腹內(nèi)脂肪厚度,分析肝前脂肪厚度、腹內(nèi)脂肪厚度與腰圍之間的相關(guān)性,并用受試者特征(ROC)曲線分析肝前脂肪厚度診斷代謝綜合征的最佳臨界值。結(jié)果:肝前脂肪厚度、腹內(nèi)脂肪厚度均與腰圍呈正相關(guān)(P0.01);肝前脂肪厚度與腰圍的相關(guān)系數(shù)大于腹內(nèi)脂肪厚度與腰圍的相關(guān)系數(shù)。男性肝前脂肪厚度診斷代謝綜合征的最佳臨界值為1.30 cm,敏感度為0.70,特異度為0.81,ROC曲線下面積為0.82(P0.01);女性肝前脂肪厚度診斷代謝綜合征的最佳臨界值為1.27 cm,敏感度為0.88,特異度為0.74,ROC曲線下面積為0.87(P0.01)。結(jié)論:超聲測(cè)量肝前脂肪厚度可替代腰圍,作為評(píng)價(jià)腹內(nèi)脂肪堆積程度的準(zhǔn)確診斷代謝綜合征的指標(biāo)之一。
[Abstract]:Objective: to evaluate the value of ultrasonic measurement of prehepatic fat thickness in the diagnosis of metabolic syndrome. Methods: 496 patients with endocrine department were divided into groups according to the related diseases of metabolic syndrome. The thickness of prehepatic fat and abdominal fat were measured by ultrasound, and the thickness of prehepatic fat was analyzed. The correlation between abdominal fat thickness and waist circumference. The best critical value of prehepatic fat thickness for the diagnosis of metabolic syndrome was analyzed by using the characteristic ROC curve. Results: the correlation coefficient between prehepatic fat thickness and waist circumference was higher than that between abdominal fat thickness and waist circumference. The best critical value for the diagnosis of metabolic syndrome was 1.30 cm for male prehepatic fat thickness, the sensitivity was 0.70, and the specificity was 0.81g ROC curve for the area under 0.82mP0.010.The best critical value for the diagnosis of metabolic syndrome was 1.27 cm and the sensitivity was 0.88 cm for female prehepatic fat thickness. The area under the ROC curve was 0.87 (P 0.01). Conclusion: the measurement of prehepatic fat thickness by ultrasound can replace waist circumference and be used as an index to evaluate the degree of abdominal fat accumulation in the diagnosis of metabolic syndrome.
