【摘要】：目的探討超聲對銀屑病關(guān)節(jié)炎(PsA)的診斷價值。方法收集我院門診及住院PsA患者32例,所有患者均接受超聲及體格檢查,包括雙側(cè)手足小關(guān)節(jié)、指甲及下肢主要附著點(diǎn)(股四頭肌腱、髕韌帶、跟腱及跖筋膜附著端)等,分別觀察并計數(shù)上述關(guān)節(jié)、附著點(diǎn)及指甲受累情況,并進(jìn)行統(tǒng)計學(xué)分析。結(jié)果體格檢查發(fā)現(xiàn)406個(406/1792,22.66%)關(guān)節(jié)病變,79個(79/320,24.69%)附著點(diǎn)炎,84個(84/320,26.25%)指甲病變;超聲檢查發(fā)現(xiàn)492個(492/1792,27.46%)關(guān)節(jié)病變,166個(166/320,51.88%)附著點(diǎn)炎,203個(203/320,63.44%)指甲病變;超聲檢查陽性率均高于體格檢查(P均0.001)。結(jié)論超聲能早于體格檢查發(fā)現(xiàn)PsA患者關(guān)節(jié)、附著點(diǎn)及指甲病變,超聲對PsA的診斷具有重要價值。
[Abstract]:Objective to investigate the diagnostic value of ultrasound in psoriatic arthritis (PsA). Methods 32 cases of PsA were collected. All the patients received ultrasound and physical examination, including bilateral facet joints of hand and foot, main attachment points of nail and lower extremity (quadriceps femoris tendon, patellar ligament, Achilles tendon and metatarsal fascia attachment). The joint, attachment point and nail involvement were observed and counted respectively and analyzed statistically. Results physical examination revealed 406 lesions of 406 / 1792x 22.66) of joints, 79 of 79 / 320 / 320-24.69) attachment point inflammation, 84 / 84 / 320,26.25) of nail lesions; 492 lesions of 492 / 492 / 1792or 27.46) joints, 166 / 166 / 32051.88) adhesion points, 203 / 203 / 203 / 320 / 63.44) nail lesions; the positive rate of ultrasound examination was all higher than that of physical examination (P = 0.001, P = 0.001, P < 0. 001respectively), and the positive rate of ultrasound examination was higher than that of physical examination (P < 0. 001P = 0. 001P = 0. 001P = 0. 001P = 0. 001P = 0. 001P = 0. 001P). Conclusion Ultrasound can find joint, attachment and nail lesions in PsA patients earlier than physical examination. Ultrasound has important value in the diagnosis of PsA.
