【摘要】：目的：探討原發(fā)性高血壓患者與主動(dòng)脈重構(gòu)和左房重構(gòu)相關(guān)的諸因素變化特點(diǎn)及相互關(guān)系。方法：隨機(jī)抽查在新疆醫(yī)科大學(xué)第一附屬醫(yī)院診斷為原發(fā)性高血壓的924例住院患者,根據(jù)主動(dòng)脈及左房是否重構(gòu)擴(kuò)大分為主動(dòng)脈增寬左房增大組、主動(dòng)脈增寬左房正常組、左房增大主動(dòng)脈正常組、主動(dòng)脈正常左房正常組,分別測(cè)量各組患者的左房收縮末期內(nèi)徑(LADs)、主動(dòng)脈竇部?jī)?nèi)徑、左心室舒張末期內(nèi)徑(LVEDd),舒張期室間隔及左室后壁厚度(IVSd、LVPWd)并計(jì)算左心室質(zhì)量指數(shù)(LVMI)心臟指標(biāo)。另外,測(cè)定924例住院患者的年齡、性別,體重指數(shù)(BMI),收縮壓,舒張壓,病程及治療時(shí)間等其他指標(biāo)；運(yùn)用t檢驗(yàn),卡方檢驗(yàn)分析上述指標(biāo)的差異,用logistics回歸方法分析上述因素對(duì)主動(dòng)脈重構(gòu)和左心房重構(gòu)的影響,并做ROC曲線計(jì)算引起主動(dòng)脈重構(gòu)和左心房重構(gòu)最重要的指標(biāo)。結(jié)果：1.主動(dòng)脈與左房均重構(gòu)組的年齡、BMI、LVMI、收縮壓、舒張壓、病程均高于兩者未重構(gòu)組,LVMI、收縮壓、舒張壓均高于主動(dòng)脈增寬左房正常組(P0.05)。2.主動(dòng)脈增寬左房正常組的年齡、LVMI、收縮壓、病程高于主動(dòng)脈正常左房正常組(P0.05),其年齡高于主動(dòng)脈正常左房增大組,治療時(shí)間短于主動(dòng)脈正常左房正常組。3.主動(dòng)脈正常左房增大組的年齡、BMI、LVMI、舒張壓均高于主動(dòng)脈正常左房正常組,治療時(shí)間短于主動(dòng)脈正常左房正常組。結(jié)論：年齡是引起主動(dòng)脈重構(gòu)的重要影響因素,而LVMI是引起左心房重構(gòu)的重要影響因素。
[Abstract]:Objective: To investigate the characteristics and relationship of various factors associated with aortic remodeling and left atrial remodeling in patients with essential hypertension. Methods: 924 hospitalized patients diagnosed as primary hypertension at the First Affiliated Hospital of Xinjiang Medical University were randomly selected to be divided into aorta and left atrium by enlargement of aorta and left atrium to the widening of the left atrium. In the large group, the aorta widened the left atrial normal group, the left atrial enlargement of the normal group, the normal left atrium of the aorta, the left atrial end systolic diameter (LADs), the internal diameter of the aortic sinus, the left ventricular end diastolic diameter (LVEDd), the diastolic interventricular septum and the left ventricular posterior wall thickness (IVSd, LVPWd), and the left ventricular mass index (LV). MI) cardiac index. In addition, the age, sex, body mass index (BMI), systolic pressure, diastolic pressure, course of disease and treatment time were measured in 924 hospitalized patients. The differences of the above indexes were analyzed by t test and chi square test. The effects of these factors on the remodeling of aorta and left atrium were analyzed with the logistics regression method, and the ROC curve was made. Results: 1. the age, BMI, LVMI, systolic pressure, diastolic pressure, and diastolic pressure of both aorta and left atrial remodeling group were higher than those in the two groups, LVMI, systolic pressure and diastolic pressure were higher than that of the aorta widened left atrial normal group (P0.05),.2. aorta widened the normal group of left atrium, LVMI. The course of contraction is higher than that of normal left atrium of aorta (P0.05). The age is higher than that of normal left atrium. The treatment time is shorter than that of normal left atrium of the aorta, normal left atrial enlargement group of.3. aorta, BMI, LVMI, and the diastolic pressure is higher than that of normal left atrium of aorta, and the time of treatment is shorter than normal left atrium of aorta. Conclusion: age is an important factor affecting aortic remodeling, and LVMI is an important factor affecting left atrial remodeling.
