呼吸睡眠暫停低通氣患者腦功能的功能性MRI診斷

【摘要】:目的:探討靜息狀態(tài)下呼吸睡眠暫停低通氣(OSAHS)患者腦功能是否發(fā)生異常及經(jīng)持續(xù)正壓通氣(CAPP)治療后腦功能的改變情況。方法:搜集25例中重度OSAHS患者(患者組)及25例健康志愿者(對照組)。患者CAPP治療前后及健康志愿者均進(jìn)行MRI平掃及血氧依賴水平功能性磁共振成像(BOLD-fMRI)檢查。結(jié)果:與對照組相比,患者組治療前右側(cè)額中回等腦區(qū)的局部一致性減低,右側(cè)島葉等腦區(qū)的低頻振幅(ALFF)值減低,右側(cè)額下回等腦區(qū)的低頻振幅比率(fALFF)值減低。與對照組相比,患者組治療后右側(cè)小腦半球等腦區(qū)的局部一致性減低,右側(cè)楔葉等腦區(qū)的ALFF減低,右側(cè)額下回等腦區(qū)的fALFF減低。與治療前相比,患者組治療后右側(cè)顳下回等腦區(qū)的局部一致性明顯增高,右側(cè)小腦半球等腦區(qū)的ALFF增高,左側(cè)枕中回等腦區(qū)的fALFF增高。結(jié)論:靜息狀態(tài)下OSAHS患者腦功能狀態(tài)存在異常,經(jīng)CAPP治療后腦功能異常的腦區(qū)可部分逆轉(zhuǎn)。功能性MRI對呼吸睡眠暫停低通氣患者腦功能的診斷具有重要價值。
[Abstract]:Objective: to investigate the abnormal brain function in patients with sleep apnea hypopnea (OSAHS) and the changes of brain function after continuous positive pressure ventilation (CPAP) treatment. Methods: 25 moderate and severe OSAHS patients (patient group) and 25 healthy volunteers (control group) were collected. MRI plain scan and blood oxygen dependent level functional magnetic resonance imaging (BOLD-fMRI) were performed before and after CAPP treatment in healthy volunteers. Results: compared with the control group, the local consistency of the right middle frontal gyrus isencephalon, the low frequency amplitude of the right isobaric area and the ratio of low frequency amplitude of the right inferior frontal gyrus to the isencephalic area were decreased before treatment, and the ratio of the low frequency amplitude of the right inferior frontal gyrus to the isencephalon was lower than that of the control group. Compared with the control group, the local consistency of isencephalic region in the right cerebellar hemisphere was decreased, the ALFF in the right wedge lobe and the fALFF in the right inferior frontal gyrus were decreased in the patient group. After treatment, the local consistency of the right inferior temporal gyrus was significantly increased, the ALFF of the right cerebellar hemisphere was increased, and the fALFF of the left middle occipital gyrus was increased. Conclusion: there is abnormal brain function in patients with OSAHS at rest, and the brain regions with abnormal brain function after CAPP treatment can be partially reversed. Functional MRI plays an important role in the diagnosis of brain function in patients with apnea hypopnea.
