【摘要】：目的應(yīng)用實(shí)時(shí)三維超聲心動(dòng)圖(RT-3DE)技術(shù),并通過(guò)與二維超聲心動(dòng)圖(2DE)比較,探討其在診斷結(jié)構(gòu)性心臟病中的應(yīng)用價(jià)值。方法選取2009年10月至2014年2月在南昌大學(xué)第一附屬醫(yī)院住院的92例心臟病患者,全部患者進(jìn)行RT-3DE檢查,通過(guò)對(duì)感興趣區(qū)病變結(jié)構(gòu)三維圖像進(jìn)行切割及旋轉(zhuǎn)后處理作出診斷,同時(shí)對(duì)房間隔缺損(ASD)及室間隔缺損(VSD)大小進(jìn)行測(cè)量分析,以外科手術(shù)結(jié)果為金標(biāo)準(zhǔn),對(duì)比2DE與RT-3DE在結(jié)構(gòu)性心臟病診斷率的一致性和差異,評(píng)價(jià)RT-3DE準(zhǔn)確性和優(yōu)越性。結(jié)果 (1)RT-3DE對(duì)92例心臟病患者均成功采集三維圖像,對(duì)病變結(jié)構(gòu)能立體、直觀地從各個(gè)角度得到顯示,正確診斷病變的部位、范圍和程度及與鄰近組織結(jié)構(gòu)的空間位置關(guān)系,獲得2DE無(wú)法得到的切面觀,較2DE提供了更為直觀而準(zhǔn)確的信息。(2)在先天性心臟病中,RT-3DE診斷符合率與2DE相同;在瓣膜病及其他病變中,RT-3DE則稍高于2DE。(3)與2DE所測(cè)直徑相比,RT-3DE所測(cè)ASD最大長(zhǎng)軸徑與手術(shù)結(jié)果具有更好的相關(guān)性(r值分別為0.94和0.86);RT-3DE所測(cè)VSD最大長(zhǎng)軸徑與手術(shù)結(jié)果具有更好的相關(guān)性(r值分別為0.93和0.84)。結(jié)論 RT-3DE操作簡(jiǎn)便,成像速度快,在結(jié)構(gòu)性心臟病的診斷中可獲得比2DE更豐富的空間信息,提高了診斷正確率,可作為2DE的有益補(bǔ)充,在結(jié)構(gòu)性心臟病外科手術(shù)方案的制定上具有指導(dǎo)意義。
[Abstract]:Objective to evaluate the value of real-time three-dimensional echocardiography (RT-3DE) in the diagnosis of structural heart disease by comparing it with two-dimensional echocardiography (2DE). Methods 92 patients with heart disease were selected from October 2009 to February 2014 in the first affiliated Hospital of Nanchang University. All the patients were examined by RT-3DE, and the diagnosis was made by cutting and rotating 3D images of lesion structure in the region of interest. The size of atrial septal defect (ASD) and ventricular septal defect (VSD) were measured and analyzed. The accuracy and superiority of RT-3DE were evaluated by comparing the accuracy and difference of 2DE and RT-3DE in the diagnosis of structural heart disease. Results Third-dimensional images were collected successfully from 92 patients with heart disease by RT-3DE. The lesions were stereoscopically and intuitively displayed from various angles. The location, extent and extent of the lesions were correctly diagnosed, and the spatial relationship with the adjacent tissue structure was obtained. In congenital heart disease, the diagnostic coincidence rate of RT-3DE was the same as that of 2DE. In valvular diseases and other diseases, RT-3DE was slightly higher than 2DE.3DE) compared with the diameter measured by 2DE, the maximum long axis diameter of ASD measured by RT-3DE was better correlated with the operative results. The r values of RT-3DE were 0.94 and 0.86 respectively. The maximum length diameter of VSD measured by RT-3DE was better than that of operation. The good correlation r values were 0.93 and 0.84g, respectively. Conclusion RT-3DE has the advantages of simple operation and fast imaging. It can obtain more spatial information than 2DE in the diagnosis of structural heart disease, and improve the diagnostic accuracy. It can be used as a useful supplement to 2DE. It is of guiding significance in the formulation of surgical procedures for structural heart disease.
