VBM對首發(fā)未經(jīng)治療抑郁癥患者腦結(jié)構(gòu)改變的研究

【摘要】:[目的]運(yùn)用基于體素的形態(tài)學(xué)測量方法分析首發(fā)未經(jīng)治療抑郁癥患者腦灰質(zhì)變化,探討抑郁癥腦結(jié)構(gòu)改變的神經(jīng)病理生理學(xué)機(jī)制。[方法]收集首發(fā)抑郁癥患者27例(MDD組)及與MDD組性別、年齡及受教育程度匹配的健康志愿者30名;所有患者由兩名以上臨床心理科醫(yī)生根據(jù)美國《精神疾病診斷與統(tǒng)計手冊》第四版(Diagnostic and statistical manual,fourth edition DSM-IV)診斷標(biāo)準(zhǔn)確診。采用美國GE公司1.5T Signa(HDXT)雙梯度磁共振掃描儀,頭部相控陣線圈進(jìn)行常規(guī)頭顱磁共振、全腦3D結(jié)構(gòu)及功能磁共振掃描,獲取被試者全腦相應(yīng)數(shù)據(jù),采用基于體素的形態(tài)測量(voxel-based morphometry,VBM)方法對兩組的腦結(jié)構(gòu)MRI圖像的灰質(zhì)體積分析,采用兩獨(dú)立樣本t檢驗(yàn)對兩組結(jié)果進(jìn)行比較,當(dāng)p0.05時認(rèn)為差異有統(tǒng)計學(xué)意義。[結(jié)果](1)抑郁癥組與健康組在性別、年齡及受教育年限上差異無統(tǒng)計學(xué)意義。(2)VBM分析顯示首發(fā)抑郁癥組較正常對照組灰質(zhì)體積減小的腦區(qū):右側(cè)顳上回、右側(cè)顳中回、右側(cè)顳下回(P0.001,cluster≥12)。增加的腦區(qū):左側(cè)額上回、左側(cè)額中回、左側(cè)額下回、左前扣帶回、右側(cè)額下回(P0.001,cluster≥12)。[結(jié)論]抑郁癥患者腦灰質(zhì)體積較健康人異常的腦區(qū),主要涉額葉、顳葉及扣帶回,可能是抑郁癥發(fā)病的神經(jīng)生理病理學(xué)機(jī)制的關(guān)鍵。
[Abstract]:[objective] to investigate the neuropathophysiological mechanism of the changes of brain structure in patients with depression without initial treatment by means of morphometric measurement based on voxel. [methods] 27 patients with first-episode depression (MDD group) and 30 healthy volunteers matched with sex, age and education level of MDD group were collected. All patients were diagnosed by more than two clinical cardiologists according to the diagnostic criteria of Diagnostic and statistical fourth edition DSM-IV. Two gradient magnetic resonance scanners of 1.5T SignaAn HDXT of GE Company were used in this study. The head phased array coil was used to perform conventional cranial magnetic resonance imaging, and 3D structure and functional magnetic resonance scanning of the whole brain were performed to obtain the corresponding data of the whole brain of the subjects. A voxel-based morphometrymetric method was used to analyze the gray matter volume of MRI images of brain structure in two groups. Two independent samples t-test were used to compare the results of the two groups. When p0.05, the difference was statistically significant. [results] there was no significant difference between the depression group and the healthy group in sex, age and years of education. VBM analysis showed that the gray matter volume of the first episode depression group was smaller than that of the normal control group: right superior temporal gyrus, right middle temporal gyrus, right superior temporal gyrus, and right middle temporal gyrus.
