【摘要】：目的評價MRI彌散張量成像(DTI)在腎透明細(xì)胞癌(ccRCC)與腎盂移行細(xì)胞癌(TCC)鑒別診斷中的價值。方法回顧性分析行腹部MR檢查、經(jīng)病理證實為ccRCC及TCC的患者38例(ccRCC 29例,TCC 9例)�；颊呔蠱R T1W脂肪抑制和T2W脂肪抑制序列掃描、LAVA增強(qiáng)掃描、DTI序列掃描(b=0、600s/mm~2)。由2名放射科醫(yī)師采用AW 4.4工作站采用Functool后處理軟件進(jìn)行圖像分析和測量。采用組內(nèi)相關(guān)系數(shù)(ICC)檢驗2名察者所測數(shù)據(jù)的一致性。ccRCC和TCC ADC值及FA值的比較采用獨立樣本t檢驗。采用ROC曲線分析ADC值、FA值對ccRCC與TCC的鑒別診斷效能。結(jié)果 2名觀察者測量各參數(shù)一致性良好(ICC值均0.75)。ccRCC的ADC值[(2.03±0.49)×10~(-3) mm~2/s]高于TCC[(1.57±0.43)×10~(-3 )mm~2/s;P=0.015)],但ccRCC的FA值(0.24±0.10)低于TCC(0.42±0.22);P=0.002)。ADC值曲線下面積0.761(P0.05),敏感度和特異度分別為79.3%、77.8%,閾值為1.59×10~(-3) mm~2/s。FA值曲線下面積為0.762(P0.05),敏感度和特異度分別為66.7%、93.1%,閾值為0.326。結(jié)論 MR DTI可有效鑒別ccRCC和腎盂TCC,其中FA值對鑒別兩者的特異度較高。
[Abstract]:Objective to evaluate the value of MRI diffusion Zhang Liang imaging in differential diagnosis of renal clear cell carcinoma (RCC) and transitional cell carcinoma of renal pelvis (TCC). Methods the abdominal Mr examination was performed retrospectively in 38 patients with ccRCC and TCC confirmed by pathology. There were 29 patients with ccRCC and 9 patients with TCCs. Mr T1W fat suppression and T2W fat suppression sequences were performed in all patients. Two radiologists used AW 4.4 workstation to carry out image analysis and measurement with Functool post processing software. The consistency of the data measured by the two subjects was compared with the values of TCC ADC and FA. The independent sample t test was used to test the consistency of the data.
