【摘要】：目的研究分析重癥全身性感染患者急性腎損害(AKI)超聲早期診斷的臨床評(píng)價(jià),為急性腎損害的確診提供依據(jù)。方法選擇2010年10月-2015年10月于醫(yī)院住院治療的重癥感染患者62例進(jìn)行研究,根據(jù)第7天的超聲診斷結(jié)果及血肌酐(Scr)檢測(cè)結(jié)果進(jìn)行分組,合并AKI患者30例為觀察組,未合并AKI患者32例為對(duì)照組,對(duì)比兩組患者相關(guān)指標(biāo)水平,數(shù)據(jù)采用SPSS 20.0軟件進(jìn)行統(tǒng)計(jì)分析。結(jié)果觀察組在第1天及第7天的腎葉間動(dòng)脈阻力指數(shù)(RI)均顯著高于對(duì)照組同時(shí)期的水平;觀察組在第7天的Scr水平顯著高于第1天及對(duì)照組第7天時(shí)的水平;超聲檢查真陽性28例,真陰性29例;Scr檢測(cè)真陽性21例,真陰性20例;早期超聲檢查的敏感度、特異度及準(zhǔn)確度均分別高于Scr檢測(cè),差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論利用超聲在早期時(shí)評(píng)價(jià)重癥全身性感染者是否合并AKI具有較高的準(zhǔn)確性,臨床應(yīng)用價(jià)值較好,值得推廣。
[Abstract]:Objective to study the clinical evaluation of early diagnosis of acute renal damage (AKI) in patients with severe systemic infection and to provide evidence for the diagnosis of acute renal damage. Methods Sixty-two patients with severe infection who were hospitalized in hospital from October 2010 to October 2015 were studied. According to the results of ultrasound diagnosis and creatinine (creatinine) detection on the 7th day, 30 patients with AKI were divided into two groups. 32 patients without AKI were taken as control group. The data were analyzed by SPSS 20.0 software. Results the resistance index (RI) of the renal interlobar artery in the observation group was significantly higher than that in the control group on the 1st and 7th day, and the level of Scr in the observation group on the 7th day was significantly higher than that in the control group on the 7th day and on the 7th day in the control group. The sensitivity, specificity and accuracy of early ultrasound examination were higher than that of Scr, and the difference was statistically significant (P 0.05). Conclusion Ultrasonography has high accuracy in evaluating severe systemic infection with AKI in the early stage, and is worth popularizing because of its good clinical value.
