糖尿病心臟構型超聲數(shù)據(jù)模型的初步研究

【摘要】:目的:利用超聲心動圖檢查獲取糖尿病患者的心臟構型基礎數(shù)據(jù),并結合相關參數(shù)創(chuàng)建糖尿病心臟構型超聲數(shù)據(jù)倉庫,利用數(shù)據(jù)挖掘技術構建心臟構型超聲數(shù)據(jù)模型,揭示糖尿病心臟構型的特征及變化規(guī)律或趨勢,從基礎數(shù)據(jù)中優(yōu)選出反映其心臟構型及其變化的特征性指標。 方法:選取糖尿病患者360例及正常健康人150例為研究對象,記錄受試者的身高、體重及體表面積等形體指標和相關指標的影響因素如性別、年齡、血壓、心率、血脂、血糖等。于標準超聲心動圖切面,采集受檢者心臟構型超聲檢查基礎數(shù)據(jù),包括室間隔厚度、左室后壁厚度、升主動脈內(nèi)徑、心肌重量、各房室腔徑、面積、容積等。將測量的心臟構型各項指標、形體指標與相關指標的影響因素相結合,建立心臟構型超聲數(shù)據(jù)倉庫及數(shù)據(jù)模型,然后根據(jù)各指標權重系數(shù)的大小從基礎數(shù)據(jù)中優(yōu)選出反映糖尿病心臟構型及其變化的特征性指標。 結果:建立了正常人和糖尿病人群的心臟構型超聲數(shù)據(jù)倉庫及數(shù)據(jù)模型,并繪制出趨勢圖;通過對糖尿病心臟構型的各項指標與相關影響因素進行分析,根據(jù)各指標權重系數(shù)的大小,,顯示左房收縮期末前后徑、左房收縮期末面積、左房容積、左房容積指數(shù)、左室心肌重量、左室心肌重量指數(shù)、室壁相對厚度、室間隔厚度、左室后壁厚度在糖尿病時傾向于發(fā)生改變。 結論:左房收縮期末前后徑、左房收縮期末面積、左房容積、左房容積指數(shù)、左室心肌重量、左室心肌重量指數(shù)、室壁相對厚度、室間隔厚度、左室后壁厚度可以作為反映糖尿病心臟構型改變的特征性指標,以實現(xiàn)對糖尿病心臟構型演變的動態(tài)監(jiān)測和中長期預報,為糖尿病的防治提供決策支持。
[Abstract]:Objective: to obtain the basic data of heart configuration of diabetic patients by echocardiography, and to establish the data warehouse of heart configuration of diabetes mellitus by combining the relevant parameters, and to construct the echocardiographic data model of heart configuration by using data mining technology. To reveal the characteristics of diabetic heart configuration and its changing law or trend, and to select the characteristic indexes from the basic data to reflect the heart configuration and its changes. Methods: 360 diabetic patients and 150 normal controls were selected to record the body height, body weight, body surface area and related factors such as sex, age, blood pressure, heart rate, blood lipid, blood sugar and so on. The basic data of echocardiography were collected, including the thickness of interventricular septum, the thickness of posterior wall of left ventricle, the diameter of ascending aorta, the weight of myocardium, the diameter of atrioventricular cavity, the area and volume of each atrioventricular cavity. The ultrasonic data warehouse and data model of heart configuration were established by combining the measured cardiac configuration indexes, body shape indexes and the influencing factors of related indexes. Then, according to the weight coefficient of each index, the characteristic indexes reflecting the heart configuration and changes of diabetes mellitus were selected from the basic data. Results: the echocardiographic data warehouse and data model of heart configuration in normal subjects and diabetic population were established, and the trend map was drawn. According to the weight coefficient of each index, the left atrial end systolic diameter, the area of left atrial systolic end, the left atrial volume index, the left ventricular mass index, the relative thickness of ventricular wall, the thickness of interventricular septum, the left atrial volume index, the left atrial systolic area, the relative thickness of ventricular wall, the thickness of interventricular septum were shown. Left ventricular posterior wall thickness tends to change in diabetes. Conclusion: left atrial end systolic diameter, left atrial end systolic area, left atrial volume, left atrial volume index, left ventricular mass index, left ventricular mass index, relative thickness of ventricular wall, thickness of interventricular septum, left atrial volume index, left ventricular end systolic diameter, left atrial volume, left ventricular volume index, left ventricular mass index, relative thickness of ventricular wall The left ventricular posterior wall thickness can be used as a characteristic index to reflect the change of diabetic heart configuration in order to realize the dynamic monitoring and medium and long term prediction of diabetic heart configuration evolution and provide decision support for the prevention and treatment of diabetes mellitus.
