【摘要】：目的探討胸中上段食管癌調(diào)強(qiáng)放療(IMRT)不同能量的X線對(duì)腫瘤計(jì)劃靶區(qū)(PTV)及危及器官的影響。方法 96例胸中上段食管癌患者,在Pinnacle3治療計(jì)劃系統(tǒng)中分別采用6 MV和15 MV X線對(duì)每例患者進(jìn)行調(diào)強(qiáng)放療計(jì)劃設(shè)計(jì)。兩組IMRT優(yōu)化參數(shù)和射野參數(shù)相同,分析比較不同能量X線放療條件下患者PTV的劑量和體積分布參數(shù)、均勻性指數(shù)、適形度指數(shù)以及雙肺、脊髓、心臟受照射劑量的差異。結(jié)果經(jīng)不同能量X線治療后,6 MV組與15 MV組PTV中Dmin、Dmax、Dmean、D5%、D95%、HI及CI均無顯著差異(P0.05);而15 MV組肺V5、肺V20、肺V30、脊髓Dma、心臟V30、心臟Dmean、正常組織V20及正常組織V31.5等參數(shù)明顯高于6 MV組(P0.05),而肺V10、肺V50、肺Dmean、心臟V40、正常組織V5、正常組織V10指標(biāo)差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論針對(duì)胸中上段食管癌患者,采用6 MV X線進(jìn)行IMRT能夠有效減少放療過程中對(duì)危及器官及正常組織的放射性損傷,降低相關(guān)并發(fā)癥發(fā)生率。
[Abstract]:Objective to investigate the effect of different energy of IMRTT on PTV (tumor planning target) and organ danger in upper and middle thoracic esophageal carcinoma. Methods 96 patients with upper and middle thoracic esophageal carcinoma were treated with 6 MV and 15 MV X-ray in the Pinnacle3 treatment planning system respectively. The intensity modulated radiotherapy plan was designed for each patient. The optimal parameters and field parameters of IMRT were the same in two groups. The dose and volume distribution parameters, uniformity index, conformability index and dose of radiation to the lungs, spinal cord and heart of patients with PTV were analyzed and compared under different energy X-ray radiation conditions. Results after different energy X ray therapy, there was no significant difference in DmaxDmaxDmean D5 and D95HI and CI in PTV between 6 MV group and 15 MV group, while the parameters of lung V5, lung V20, lung V30, spinal cord Dma, heart V30, heart Dmean, normal tissue V20 and normal tissue V31.5 in 15 MV group were significantly higher than those in 6 MV group. There was no significant difference in V10, V50, Dmean, V40, V5 and V10 between the two groups. Conclusion for patients with middle and upper thoracic esophageal carcinoma, IMRT with 6 MV X ray can effectively reduce radiation damage to organs and normal tissues during radiotherapy, and reduce the incidence of related complications.
