【摘要】：目的評(píng)價(jià)磁共振靜脈成像(magnetic resonance venography,MRV)顯示小腦幕腦膜瘤侵犯靜脈竇的準(zhǔn)確性,探討MRV在小腦幕腦膜瘤術(shù)中的指導(dǎo)意義。方法回顧分析2009年10月~2014年4月我科經(jīng)顯微手術(shù)治療的小腦幕腦膜瘤36例資料,術(shù)前均行頭顱MRV,成像技術(shù)包括無需造影劑的時(shí)間飛躍法和相位對(duì)比法,以及需造影劑的對(duì)比增強(qiáng)法,掃描后采用最大強(qiáng)度投影后處理成像,部分靜脈區(qū)域根據(jù)需要加做多平面重建成像,最后觀察靜脈竇的顯示情況及其與小腦幕腦膜瘤的關(guān)系,并與術(shù)中所見結(jié)果對(duì)比。結(jié)果 36例小腦幕腦膜瘤患者中,MRV對(duì)靜脈竇受侵部位的顯示與術(shù)中所見完全一致;MRV診斷靜脈竇受侵犯的敏感性為92.6%(25/27),特異性為100%(9/9),準(zhǔn)確率為94.4%(34/36)。MRV對(duì)小腦幕腦膜瘤相關(guān)側(cè)支靜脈吻合顯示的敏感性為42.9%(9/21),特異性為86.7%(13/15),準(zhǔn)確率為61.1%(22/36)。結(jié)論MRV可準(zhǔn)確地顯示小腦幕腦膜瘤侵犯靜脈竇的部位及程度,指導(dǎo)手術(shù)方案的選擇,具有重要臨床意義。
[Abstract]:Objective to evaluate the accuracy of magnetic resonance resonance venography (resonance) in showing the involvement of tentorial meningiomas into the venous sinuses, and to explore the guiding significance of MRV in the operation of tentorial meningiomas. Methods from October 2009 to April 2014, 36 cases of cerebellar tentorial meningioma treated by microsurgery were retrospectively analyzed. MRVs were performed before operation. The imaging techniques included time leapfrogging method and phase contrast method without contrast agent. The contrast enhancement method of contrast medium was used, the maximum intensity projection was used after scanning, and the multiplanar reconstruction was performed in some venous regions according to the need. Finally, the display of venous sinus and its relationship with tentorial meningioma were observed. The results were compared with the operative results. Results in 36 patients with tentorial meningioma, the sensitivity of MRV in the diagnosis of venous sinus invasion was 92. 6% 25 / 27, the specificity was 100% 9 / 9, and the accuracy rate was 94.4%(34/36).MRV for the associated side of cerebellar tentorial meningioma. The sensitivity of the anastomosis was 42.9%, the specificity was 86.7%, and the accuracy was 61.1% 22 / 36%. Conclusion MRV can accurately show the location and extent of cerebellar tentorial meningioma invading the venous sinuses and guide the choice of surgical scheme, which is of great clinical significance.
