【摘要】：目的應(yīng)用表觀擴(kuò)散系數(shù)(ADC)早期評(píng)價(jià)乳腺癌肝轉(zhuǎn)移(BCLMs)化療效果。方法搜集19例(共42個(gè)轉(zhuǎn)移灶)乳腺癌肝轉(zhuǎn)移患者資料,10例單用蒽環(huán)類治療,9例采用蒽環(huán)聯(lián)合紫衫類聯(lián)合治療。于化療前3~4天,化療后1周,化療后2周及化療后1個(gè)月行MRI平掃及擴(kuò)散加權(quán)成像(DWI),并測(cè)定ADC值。采用實(shí)體瘤療效評(píng)價(jià)標(biāo)準(zhǔn)(RECIST)于化療4周后劃分有效組與無(wú)效組。結(jié)果在42個(gè)轉(zhuǎn)移灶中,有效組有11個(gè),無(wú)效組共31個(gè),有效組化療前后ADC值分別為(0.9±0.16)×10~(-3)mm~2/s,(1.05±0.12)×10~(-3)mm~2/s,(1.26±0.12)×10~(-3)mm~2/s,(1.33±0.87)×10~(-3)mm~2/s;化療前有效組與無(wú)效組的ADC值分別為(0.9±0.16)×10~(-3)mm~2/s,(1.09±0.24)×10~(-3)mm~2/s,F=1.790,P0.05。繪制ROC曲線,化療后2周,曲線下面積為0.782其可信區(qū)間為64.5%~91.8%,具有較高的臨床診斷價(jià)值,取ADC=1.14×10~(-3)mm~2/s作為判斷化療有效與否的臨界點(diǎn)。結(jié)論 ADC可于化療后2周定量判斷BCLMs療效。
[Abstract]:Objective to evaluate the effect of BCLMs chemotherapy with apparent diffusion coefficient (ADCC) in early stage. Methods 19 cases (42 metastatic foci) of breast cancer with liver metastasis were collected. 10 cases were treated with anthracycline alone and 9 cases were treated with anthracycline combined with purpura. MRI plain scan and diffusion-weighted imaging (DWI) were performed 3 days before chemotherapy, 1 week after chemotherapy, 2 weeks after chemotherapy and 1 month after chemotherapy. ADC values were measured. Patients with solid tumor were divided into effective group and ineffective group 4 weeks after chemotherapy according to the criteria for evaluating the curative effect of solid tumor (RECIST). 緇撴灉鍦，
