磁共振擴散峰度成像在顱內(nèi)星形細胞腫瘤術(shù)前評估中的應用

【摘要】:目的:研究磁共振擴散峰度成像(DKI)在星形細胞腫瘤術(shù)前評估中的應用價值。方法:收集經(jīng)手術(shù)病理證實的星形細胞腫瘤患者25例,其中低級別腫瘤11例,高級別腫瘤14例,均進行常規(guī)MRI及DKI檢查,測量腫瘤實質(zhì)區(qū)及對側(cè)正常腦白質(zhì)區(qū)的MK及FA值。應用白質(zhì)纖維束成像技術(shù)顯示腫瘤周圍的腦白質(zhì)纖維束。結(jié)果:MK及FA值均隨腫瘤級別增高而增高,且均具有統(tǒng)計學意義(P0.05)。白質(zhì)纖維束成像顯示:低級別腫瘤周圍白質(zhì)纖維束以受壓移位為主。高級別腫瘤周圍白質(zhì)纖維束以受浸潤及破壞為主。結(jié)論:磁共振擴散峰度成像獲得的星形細胞腫瘤實質(zhì)區(qū)的MK及FA值有助于腫瘤級別的判定,聯(lián)合應用白質(zhì)纖維束成像技術(shù)可對患者更好的進行術(shù)前評估,優(yōu)化治療方案。
[Abstract]:Objective: to study the value of Mr diffusion kurtosis imaging (DKI) in preoperative evaluation of astrocytic tumors. Methods: Twenty-five cases of astrocytoma proved by operation and pathology were collected, including 11 cases of low grade tumors and 14 cases of high grade tumors. Routine MRI and DKI examinations were performed to measure the MK and FA values in the parenchymal and contralateral white matter areas of the brain. The white matter bundle imaging technique was used to display the white matter bundle around the tumor. Results the ratio of MK and FA increased with the increase of tumor grade, and both had statistical significance (P 0.05). White matter fiber bundle imaging showed that the white matter bundle around low grade tumor was mainly displaced by compression. The white matter bundles around high grade tumors were mainly infiltrated and destroyed. Conclusion: the MK and FA values of astrocytoma parenchyma obtained by diffusion kurtosis imaging are helpful in judging the grade of astrocytoma. The combination of white matter fiber bundle imaging can better evaluate the patients before operation and optimize the treatment plan.
