超聲影像和病理測量甲狀腺乳頭狀癌的差異

【摘要】:目的比較超聲和病理測量甲狀腺乳頭狀癌尺寸的差異,討論引起差異的原因。方法回顧性分析2012年12月至2013年5月在北京協(xié)和醫(yī)院接受術(shù)前甲狀腺超聲并經(jīng)手術(shù)病理證實(shí)為甲狀腺乳頭狀癌的患者148例,按照結(jié)節(jié)的大小、形態(tài)、邊界、是否含有囊性成分以及是否合并橋本氏病分成不同亞組比較超聲和病理的測量差異。結(jié)果甲狀腺癌結(jié)節(jié)最大徑的超聲測量結(jié)果為(1.58±0.94)cm,明顯高于病理測量的(1.33±0.84)cm(P=0.000)。70.9%(105/148)結(jié)節(jié)超聲測量結(jié)果大于病理測量結(jié)果,17.6%(26/148)結(jié)節(jié)超聲測量結(jié)果小于病理測量結(jié)果。1.1~1.4 cm組最大徑的超聲和病理測量結(jié)果間差異無統(tǒng)計(jì)學(xué)意義[(1.21±0.11)cm比(1.11±0.32)cm,P=0.062],0.1~1.0 cm組最大徑的超聲測量結(jié)果為(0.75±0.19)cm,明顯高于病理測量的(0.62±0.23)cm(P=0.000),≥1.5 cm組最大徑的超聲測量結(jié)果為(2.48±0.70)cm,也明顯高于病理測量的(2.03±0.81)cm(P=0.000)。結(jié)論超聲和病理測量甲狀腺乳頭狀癌尺寸存在顯著差異,1.1~1.4 cm結(jié)節(jié)測量結(jié)果更易一致。
[Abstract]:Objective to compare the size difference of thyroid papillary carcinoma measured by ultrasound and pathology, and discuss the causes of the difference. Methods from December 2012 to May 2013, 148 patients with papillary thyroid carcinoma underwent preoperative thyroid ultrasound and confirmed by surgery and pathology, according to the size, shape and boundary of the nodules. Sonographic and pathological differences were compared between different subgroups of Hashimoto's disease, whether they contained cystic components or not. Results the maximum diameter of thyroid carcinoma was 1.58 鹵0.94 cm, which was significantly higher than that of pathological measurement (1.33 鹵0.84 cm). 70.910 5 / 148) the results of ultrasonic measurement of thyroid carcinoma were larger than that of pathological measurement (17.6% 26 / 148).
