天津地區(qū)低劑量CT肺癌基線篩查與隨訪的初步臨床分析

【摘要】:目的:探討低劑量CT(low-dose computed tomography,LDCT)基線與隨訪掃描的篩查效果,并對基線檢出與隨訪新發(fā)結(jié)節(jié)及其他胸部病變進行臨床分析。方法:研究納入2014年2月至2016年6月參與天津市腫瘤醫(yī)院放射科肺癌篩查試驗的650例被試者,并側(cè)重對其中完成隨訪掃描的548例被試者進行分析。研究者記錄基線篩查檢出結(jié)節(jié)以及其他病變,將其與隨訪掃描的LDCT圖像相比較,記錄其進展情況。結(jié)果:LDCT基線篩查有101例被試者為陽性,陽性率為18.4%。共檢出6例肺癌結(jié)節(jié),肺癌檢出率0.92%(6/650)。女性非吸煙者的肺癌檢出率1.59%(2/126)高于男性吸煙者1.04%(4/383),差異無統(tǒng)計學意義(P=0.624)。隨訪掃描檢出19例新發(fā)結(jié)節(jié)陽性被試者,新發(fā)結(jié)節(jié)陽性率為3.5%(19/548)。以三維水平對結(jié)節(jié)增大或減小的判定比例均高于二維水平。結(jié)論:LDCT肺癌篩查對早期肺癌的篩查效能顯著,對女性非吸煙者行LDCT肺癌篩查同樣意義重大。計算機輔助診斷系統(tǒng)(CAD)三維容積測量技術(shù),相較于二維直徑的評估更能對隨訪間隔內(nèi)結(jié)節(jié)的進展做出評價。
[Abstract]:Objective: to investigate the screening effect of baseline and follow-up scans of low dose CT(low-dose computed tomographyosis (LDCT), and to analyze the clinical results of baseline detection and follow-up of new nodules and other chest lesions.Methods: from February 2014 to June 2016, 650 patients were enrolled in the lung cancer screening test in radiology department of Tianjin Cancer Hospital, and 548 of them who completed follow-up scan were analyzed.The researchers recorded the nodules and other lesions detected by baseline screening and compared them with LDCT images from follow-up scans to record their progress.Results 101 subjects were positive in the 10 ~ (th)% LDCT baseline screening, and the positive rate was 18.4%.A total of 6 cases of lung cancer nodules were detected, the detection rate of lung cancer was 0.92%.The detection rate of lung cancer in female non-smokers was 1.59 / 126) higher than that in male smokers (1.044 / 4 / 383g). The difference was not statistically significant (P 0.624).19 new nodule positive subjects were detected by follow-up scan, the positive rate of new nodule was 3.5% and 19% 548%.The proportion of 3 D to nodular enlargement or decrease was higher than that of 2 D.Conclusion the LDCT lung cancer screening for female non-smokers is of great significance in early stage lung cancer screening as well as in the early stage of lung cancer screening.Computer aided diagnosis system (CAD) 3D volumetric measurement can be used to evaluate the progress of follow-up septal nodules better than that of 2D diameter.
