【摘要】：目的提出并驗證一種應(yīng)用于宮頸癌放療的基于精確表面劑量累加的直腸并發(fā)癥預(yù)測方法。方法回顧性采集到42例宮頸癌患者數(shù)據(jù),對各個治療分次的直腸壁進行精確點配準得到變形場,利用該變形場對直腸壁受照劑量進行變形和疊加,得到3D總劑量。將3D直腸總劑量分布映射到2D平面,分別提取出劑量體積特征和劑量幾何特征,并篩選出有顯著性差異(P0.05)的劑量分布特征,建立基于序列前向選擇算法和邏輯回歸的直腸并發(fā)癥預(yù)測模型。結(jié)果直腸壁表面配準的精度較高,4個相似度評價指標表明,配準后不同治療分次的直腸表面配準程度有顯著性提高。五折交叉驗證結(jié)果顯示準確性、敏感性、特異性和AUC分別為:79.5%,81.3%,75.0%和0.88。結(jié)論本文提出基于精確表面劑量累加的直腸并發(fā)癥預(yù)測模型具有可行性,為宮頸癌患者直腸毒性預(yù)測提供可靠的支持。
[Abstract]:Objective to propose and verify a method for predicting rectal complications based on accurate surface dose accumulation for cervical cancer radiotherapy.Methods data of 42 patients with cervical cancer were collected retrospectively. The deformation field was obtained by accurate registration of the rectum wall of each treatment grade. The total 3D dose was obtained by using the deformation field to deform and superposition the irradiated dose on the rectal wall.The 3D rectal total dose distribution was mapped to 2D plane to extract the dose volume characteristics and dose geometric characteristics, and to screen out the dose distribution characteristics with significant difference (P0.05).A prediction model of rectal complications based on sequential forward selection algorithm and logical regression was established.Results the accuracy of rectal surface registration was high. The four similarity evaluation indexes showed that the rectal surface registration degree of different treatment scores was significantly improved after registration.The accuracy, sensitivity, specificity and AUC were 75.0% and 0.88%, respectively.Conclusion the prediction model of rectal complications based on accurate surface dose accumulation is feasible and reliable for predicting rectal toxicity in patients with cervical cancer.
