【摘要】：目的比較MRI快速平衡穩(wěn)態(tài)采集(FIESTA)與單次激發(fā)快速自旋回波(SSFSE)序列診斷正常胎兒胸腺的價(jià)值。方法對(duì)366胎正常胎兒行MR檢查,采用FIESTA與SSFSE序列分別觀察胎兒胸腺的解剖和MR表現(xiàn),結(jié)果行Kappa和χ2檢驗(yàn)。于"三血管"層面測(cè)量胸腺面積(TA)、橫徑(TD),于矢狀位測(cè)量上下徑(SID),計(jì)算各孕周參數(shù)均值,擬合各參數(shù)與孕周的回歸方程。結(jié)果2名醫(yī)師對(duì)FIESTA和SSFSE序列圖像的評(píng)價(jià)結(jié)果一致性較好(Kappa=0.745、0.802,P均0.01),FIESTA和SSFSE序列圖像顯示清晰率分別為86.34%(632/732)和37.70%(276/732),差異有統(tǒng)計(jì)學(xué)意義(χ2=367.520,P0.001)。胎兒胸腺大小隨孕周延長(zhǎng)而增大,回歸方程分別為:TA=-5.80+0.35×孕周(r=0.820,P0.01);TD=-14.59+1.63×孕周(r=0.817,P0.01);SID=-9.63+1.44×孕周(r=0.778,P0.01)。結(jié)論 FIESTA較SSFSE序列可更清晰地顯示胎兒胸腺的細(xì)微結(jié)構(gòu)及其輪廓,MRI能清晰地顯示孕22周后的胎兒胸腺。
[Abstract]:Objective to compare the value of MRI fast balanced steady state acquisition (FIESTAA) and single shot fast spin echo (SSFSE) sequence in the diagnosis of normal fetal thymus.Methods the normal fetuses of 366 fetuses were examined by Mr. FIESTA and SSFSE sequences were used to observe the anatomy and Mr manifestations of fetal thymus respectively. Kappa and 蠂 2 tests were performed.The thymus area was measured on the "three vessels" plane, the transverse diameter was measured by TDV, and the upper and lower diameters were measured in sagittal position. The mean values of the parameters of each gestational week were calculated, and the regression equations between the parameters and the gestational weeks were fitted.Results the evaluation results of FIESTA and SSFSE images by two physicians were consistent. The clear rates of Festa and SSFSE images were 86.340.62 / 732 and 37.70 ~ 2766 / 732, respectively (蠂 ~ 2 367.520g / 0.001), and the difference was statistically significant (蠂 ~ 2 367.520g P 0.001).The size of fetal thymus increased with gestational weeks, and the regression equations were as follows: 1. TA-5.80 0.35 脳 gestational week, 0. 820 and P 0. 01, respectively. TDU-14.59 1. 63 脳 gestational week, r0. 817, P0. 01 and SID-9.63. 44 脳 gestational week, r778P0. 01.Conclusion FIESTA can display the fine structure and outline of fetal thymus more clearly than SSFSE sequence.
