【摘要】：目的 :對(duì)比分析多b值彌散加權(quán)成像(MB-DWI)與動(dòng)脈自旋標(biāo)記(ASL)在評(píng)價(jià)輕度認(rèn)知功能障礙(MCI)患者海馬灌注異常中的臨床應(yīng)用價(jià)值。方法:依據(jù)診斷標(biāo)準(zhǔn)與排除標(biāo)準(zhǔn),選擇MCI患者(MCI組)和認(rèn)知功能正常的老年人(NC組)各20例,進(jìn)行常規(guī)MRI平掃、MRA、ASL以及MB-DWI掃描,根據(jù)ASL-CBF圖測(cè)量雙側(cè)海馬最大rCBF_(ASL)值,并測(cè)量相應(yīng)區(qū)域MBDWI與灌注相關(guān)參數(shù)fast ADC-mono、fraction of fast ADC-mono、fast ADC-bi、fraction of fast ADC-bi值,對(duì)MCI組和NC組分別進(jìn)行相應(yīng)參數(shù)的組間統(tǒng)計(jì)學(xué)差異分析和Pearson相關(guān)性分析。結(jié)果:MCI組雙側(cè)海馬rCBF_(ASL)值較NC組降低(P0.05);MCI組雙側(cè)海馬fast ADC-mono、fraction of fast ADC-mono、fraction of fast ADC-bi值與NC組的比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),雙側(cè)海馬fast ADC-bi值較NC組減低,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);除fast ADC-bi值外,MCI組fast ADC-mono、fraction of fast ADC-mono、fraction of fast ADC-bi值與r CBFASL的相關(guān)性具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:MB-DWI和ASL在檢出MCI患者海馬的灌注改變方面具有一致性,可用于評(píng)估MCI患者灌注改變,有利于指導(dǎo)臨床。
[Abstract]:Objective: to compare and analyze the clinical value of MB-DWI and ASL in evaluating hippocampal perfusion abnormalities in patients with mild cognitive impairment (MCI).Methods: according to the diagnostic criteria and exclusion criteria, 20 MCI patients with MCI) and 20 elderly patients with normal cognitive function (NC group) were examined by conventional MRI plain scan and MB-DWI scanning. The maximum rCBFStax ASL value of bilateral hippocampus was measured according to ASL-CBF images.The corresponding regional MBDWI and perfusion related parameters fast ADC-monofraction of fast ADC-Monodo fast ADC-ADC-Monodo fractionof fast ADC-bi were measured. The statistical difference analysis and Pearson correlation analysis of the corresponding parameters between MCI group and NC group were carried out.Results compared with NC group, the value of fast ADC-bi in bilateral hippocampus of MCI group was significantly lower than that in NC group (P 0.05), and the value of fast ADC-bi in bilateral hippocampus was lower than that in NC group (P 0.05).There was no significant difference (P 0.05), except for fast ADC-bi, the correlation between the value of fast ADC-monochrome fractionation of fast ADC-monotaxa fractionation of fast ADC-bi and r CBFASL was statistically significant (P 0.05).ConclusionTwo MB-DWI and ASL are consistent in detecting the changes of hippocampal perfusion in patients with MCI, which can be used to evaluate the changes of perfusion in MCI patients and to guide clinical practice.
