【摘要】：目的:探討晚孕期各型胎盤植入MRI征象及其診斷價(jià)值。方法:回顧性分析2010年7月-2013年12月期間經(jīng)手術(shù)及病理證實(shí)27例胎盤植入孕婦的臨床資料。所有孕婦均行MRI檢查。結(jié)果:粘連型3例,植入型15例,穿透型9例,均為前置胎盤。三型胎盤厚度及胎盤內(nèi)流空血管數(shù)目分別為4.87cm/5.0;5.31cm/5.7;6.51cm/6.8。3例粘連型2例胎盤附著面呈弧形,1例呈結(jié)節(jié)狀膨出,肌層連續(xù);15例植入型11例胎盤結(jié)節(jié)狀膨出,11例肌層漸進(jìn)性消失,10例肌層異常信號(hào);9例穿透型4例胎盤結(jié)節(jié)狀膨出,5例塊狀突出,超過肌層輪廓,2例肌層漸進(jìn)性消失,7例肌層中斷,肌層信號(hào)均見異常,4例膀胱肌層信號(hào)異常。結(jié)論:常規(guī)MRI序列對(duì)胎盤植入的診斷與分型具有較高的應(yīng)用價(jià)值,部分病例存在征象重疊。
[Abstract]:Objective: to investigate the MRI signs of placenta accreta in late pregnancy and its diagnostic value.Methods: the clinical data of 27 pregnant women with placenta accreta confirmed by operation and pathology from July 2010 to December 2013 were analyzed retrospectively.All pregnant women underwent MRI examination.Results: there were 3 cases of adhesive type, 15 cases of implantation type and 9 cases of penetrating type, all of them were placenta previa.The thickness of placenta III and the number of empty blood vessels in placenta were 4.87 cm / 5.0 ~ 5.31 cm / 5.7 ~ 6.51 cm / 6.8.3 cases of adherent type, 2 cases of placental adhesion were arcuate and 1 case was nodular bulge.15 consecutive cases of myometrium accreta 11 cases of placental nodular bulge 11 cases of progressive disappearance of myometrium 10 cases of abnormal signal of myometrium 9 cases of penetrating type 4 cases of placental nodular bulge 5 cases of bulge.There were 2 cases of progressive disappearance of myometrium and 7 cases of discontinuity of myometrium, and 4 cases of abnormal signal of bladder myometrium were found in all cases.Conclusion: routine MRI sequence is of great value in the diagnosis and classification of placenta accreta, and some cases have overlapping signs.
