【摘要】：目的胎盤(pán)植入是妊娠少見(jiàn)的嚴(yán)重并發(fā)癥,影像診斷較困難,本文旨在探討胎盤(pán)植入MRI掃描中相對(duì)理想的快速成像序列,為影像診斷提供優(yōu)質(zhì)圖像。資料與方法回顧性分析經(jīng)手術(shù)病理證實(shí)為胎盤(pán)植入的21例孕婦的MRI圖像,患者分別行二維快速穩(wěn)態(tài)進(jìn)動(dòng)序列(2D-FIESTA)和單次激發(fā)快速自旋回波序列(SSFSE)掃描,比較兩種序列掃描的圖像優(yōu)良率及MRI表現(xiàn)。結(jié)果 2D-FIESTA序列及SSFSE序列的胎盤(pán)圖像優(yōu)良率分別為71.4%(15/21)、38.1%(8/21),差異有統(tǒng)計(jì)學(xué)意義(χ~2=4.790,P0.05)。2D-FIESTA序列及SSFSE序列對(duì)胎盤(pán)信號(hào)不均勻、胎盤(pán)內(nèi)條狀低信號(hào)影、胎盤(pán)內(nèi)增多增粗的血管影、子宮結(jié)合帶中斷、胎盤(pán)突入子宮肌層、胎盤(pán)突出子宮漿膜層各征象的顯示率分別為57.1%、57.1%、28.6%、61.9%、66.7%、14.2%及90.4%、71.4%、38.1%、42.9%、28.6%、6.5%,兩序列對(duì)胎盤(pán)信號(hào)不均勻及胎盤(pán)突入子宮肌層的顯示率比較,差異有統(tǒng)計(jì)學(xué)意義(χ~2=4.560、6.109,P0.05)。結(jié)論 2D-FIESTA序列對(duì)胎盤(pán)子宮的分界顯示較好,SSFSE序列對(duì)胎盤(pán)對(duì)比度的顯示較好,兩者結(jié)合能提高M(jìn)RI對(duì)胎盤(pán)植入的診斷價(jià)值。
[Abstract]:Objective placenta accreta is a rare serious complication of pregnancy, and the imaging diagnosis is difficult. The purpose of this paper is to study the relatively ideal fast imaging sequence in MRI scanning of placenta accreta, and to provide high quality image for the diagnosis of placenta accreta.Materials and methods the MRI images of 21 pregnant women with placenta accreta proved by surgery and pathology were retrospectively analyzed. The patients were scanned by 2D fast steady-state precession sequence (2D-FIESTA) and single-shot fast spin echo sequence (SSFSE).The excellent and good rate and MRI performance of the two sequences were compared.Conclusion the 2D-FIESTA sequence can display the placental uterine boundary better. The combination of the two sequences can improve the diagnostic value of MRI in placental accreta.
