【摘要】：目的探討浸潤性乳腺癌細胞增殖指數(shù)Ki-67表達與MRI形態(tài)學、表觀擴散系數(shù)(ADC)之間的相關(guān)性。方法選取2013年9月至2015年12月間85例浸潤性乳腺癌患者作為研究對象,所有患者術(shù)前均行常規(guī)MRI、擴散加權(quán)成像(DWI)及動態(tài)增強掃描,觀察乳腺癌的MRI特征并與Ki-67表達情況進行對比分析。結(jié)果 85例浸潤性乳腺癌Ki-67指數(shù)為5%~70%,其中高表達58例,低表達27例;無腋窩淋巴結(jié)轉(zhuǎn)移59例Ki-67均值為(30.08±13.342)%,伴有淋巴結(jié)轉(zhuǎn)移26例Ki-67均值為(40.19±12.999)%,兩組Ki-67指數(shù)差異性顯著(P=0.002,0.05);腫瘤伴有壞死29例Ki-67均值為(38.79±13.070)%,無壞死56例Ki-67均值為(30.27±3.632)%,兩組Ki-67指數(shù)差異性顯著(P=0.007,0.05);腫瘤邊緣清晰組與邊界毛糙組之間Ki-67表達的差異無統(tǒng)計學意義(P=0.106,0.05);Ki-67指數(shù)與腫瘤大小、ADC值之間無顯著相關(guān)性(P0.05)。結(jié)論在浸潤性乳腺癌的MRI形態(tài)學特征中,腫塊壞死及腋窩淋巴結(jié)轉(zhuǎn)移對于預(yù)測腫瘤細胞增殖指數(shù)Ki-67表達有重要意義。
[Abstract]:Objective to investigate the correlation between the expression of proliferative index (Ki-67) in invasive breast cancer cells and MRI morphology and apparent diffusion coefficient (MRI).Methods Eighty-five patients with invasive breast cancer between September 2013 and December 2015 were studied. All patients underwent routine MRI, diffusion weighted imaging (DWI) and dynamic enhanced scanning before operation.Results the Ki-67 index of 85 cases of invasive breast cancer was 5.70%, including 58 cases with high expression and 27 cases with low expression.There was no significant difference in Ki-67 expression between clear margin group and rough border group. There was no significant correlation between Ki-67 index and tumor size and ADC value.Conclusion among the morphologic features of MRI in invasive breast cancer, mass necrosis and axillary lymph node metastasis play an important role in predicting the expression of tumor cell proliferation index (Ki-67).
