【摘要】：目的:探討聲觸診組織量化(virtual touch tissue quantifi cation,VTQ)技術(shù)和由凝血酶原時(shí)間、谷氨酰轉(zhuǎn)肽酶與載脂蛋白A1組成的PGA指數(shù)無創(chuàng)評(píng)價(jià)酒精性肝纖維化程度的臨床應(yīng)用價(jià)值。方法:選長期飲酒史患者64例,應(yīng)用VTQ技術(shù)檢測肝超聲彈性,測得靶區(qū)肝VTQ值,計(jì)算PGA指數(shù)。以肝穿刺活檢病理結(jié)果為金標(biāo)準(zhǔn),將入選患者分為無明顯肝纖維化組(n=11)、肝纖維化組(n=10)、明顯肝纖維化組(n=14)、肝硬化組(n=29),比較VTQ值和PGA指數(shù)對酒精性肝纖維化的診斷價(jià)值。結(jié)果:無明顯肝纖維化組、肝纖維化組、明顯肝纖維化組、肝硬化組的VTQ值分別為(1.38±0.33),(1.49±0.30),(1.76±0.22)和(2.28±0.53)m/s;PGA指數(shù)分別為2.09±0.94,2.30±1.06,3.57±1.09和5.21±1.99。兩種技術(shù)測值均與肝纖維化分期存在相關(guān),VTQ的相關(guān)系數(shù)為0.719,PGA指數(shù)的相關(guān)系數(shù)為0.683(均P0.01)。結(jié)論:VTQ技術(shù)與PGA指數(shù)均能無創(chuàng)定量評(píng)價(jià)酒精性肝纖維化程度,VTQ技術(shù)作為實(shí)時(shí)超聲彈性成像技術(shù)較PGA指數(shù)更準(zhǔn)確,兩種檢查方法結(jié)合有助于酒精性肝纖維化的早期診斷及治療評(píng)估。
[Abstract]:Objective: to investigate the clinical application value of virtual touch tissue quantifi quantifi VTQs and PGA index composed of prothrombin time, glutamyl transpeptidase and apolipoprotein A1 in the evaluation of alcoholic liver fibrosis.Methods: 64 patients with a long history of drinking were selected. The ultrasonic elasticity of liver was measured by VTQ technique, the VTQ value of target liver was measured and the PGA index was calculated.According to the pathological results of liver biopsy as gold standard, the patients were divided into three groups: no hepatic fibrosis group (n = 11), liver fibrosis group (n = 10), liver fibrosis group (n = 14) and cirrhosis group (n = 29). The diagnostic value of VTQ and PGA index in alcoholic liver fibrosis was compared.Results: the VTQ values of no fibrosis group, hepatic fibrosis group, obvious hepatic fibrosis group and liver cirrhosis group were 1.38 鹵0.33, 1.49 鹵0.30, 1.76 鹵0.22) and 2.28 鹵0.53 m / s 路sPGA index were 2.09 鹵0.942.30 鹵1.063.57 鹵1.09 and 5.21 鹵1.99, respectively.The correlation coefficient of VTQ between the two techniques and the stage of hepatic fibrosis was 0.719 and the correlation coefficient of PGA index was 0.683 (all P 0.01).Conclusion both the VTQ technique and the PGA index can be used to evaluate the degree of alcoholic liver fibrosis. The VTQ technique is more accurate than the PGA index as a real-time ultrasound elastic imaging technique.The combination of the two methods is helpful to the early diagnosis and treatment of alcoholic liver fibrosis.
