3.0T MR正常大鼠部分腦組織切除術(shù)后DWI、DTI表現(xiàn)及其病理學(xué)基礎(chǔ)

【摘要】:目的建立正常大鼠部分腦組織切除術(shù)后模型,觀察術(shù)后不同時(shí)間點(diǎn)術(shù)區(qū)周邊腦組織磁共振(magnetic resonance,MR)彌散加權(quán)成像和彌散張量成像特點(diǎn)及演變規(guī)律,并探討其與病理學(xué)改變之間的關(guān)系。材料與方法36只清潔級成年Sprague Dawley大鼠隨機(jī)分為術(shù)后1天、7天、14天、21天、28天組及正常對照組,每組6只。麻醉劑采用10%Chloral hydrate(4ml/kg),腹腔注射,無菌條件下切除部分腦組織,對照組不做任何處理。手術(shù)組分別于術(shù)后1天、7天、14天、21天、28天行MR平掃和增強(qiáng)掃描,術(shù)后1天、7天、14天、21天行彌散加權(quán)成像和彌散張量成像,觀察術(shù)后不同時(shí)間點(diǎn)術(shù)區(qū)周邊腦組織和對照組相應(yīng)部位DWI、DTI特點(diǎn)及演變規(guī)律。MR檢查完成后處死動物,應(yīng)用光學(xué)顯微鏡觀察術(shù)后不同時(shí)間點(diǎn)術(shù)區(qū)周邊腦組織的病理學(xué)改變,免疫熒光染色法觀察相應(yīng)時(shí)間點(diǎn)術(shù)區(qū)周邊腦組織血管增生情況。結(jié)果1.影像學(xué)結(jié)果(1)手術(shù)7天、14天、21天、28天組術(shù)區(qū)周邊腦組織ADC值均大于對照組(P0.05);ADC值于術(shù)后7天出現(xiàn)最大值,與術(shù)后21天組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);7天、14天組間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。(2)手術(shù)各組術(shù)區(qū)周邊腦組織FA值均小于對照組(P0.05);FA值于術(shù)后7天降至最低,與術(shù)后1天、14天、21天組比較有統(tǒng)計(jì)學(xué)差異(P0.05),而在FA值在術(shù)后1天、14天、21天組間差別無統(tǒng)計(jì)學(xué)意義(P0.05)。(3)術(shù)后1天時(shí)即可看到術(shù)區(qū)周邊腦組織的反應(yīng)性強(qiáng)化,且強(qiáng)化一直持續(xù)到第28天。術(shù)后7天,術(shù)區(qū)周邊腦組織的強(qiáng)化程度明顯增強(qiáng),術(shù)后14天、21天、28天強(qiáng)化程度逐漸減弱,術(shù)后28天時(shí)僅表現(xiàn)為輕度強(qiáng)化;反應(yīng)性增強(qiáng)的形態(tài)呈斑片樣、環(huán)狀、半環(huán)狀、結(jié)節(jié)狀,對應(yīng)于殘腔的形狀,具有明顯的時(shí)間演變規(guī)律。2.病理學(xué)結(jié)果(1)HE染色:術(shù)后1天,術(shù)區(qū)腦組織正常結(jié)構(gòu)破壞,可見紅細(xì)胞大面積滲出;術(shù)后7天,手術(shù)切緣腦組織呈現(xiàn)明顯的凝固壞死,術(shù)區(qū)周邊炎癥細(xì)胞浸潤,可見大量新生毛細(xì)血管;術(shù)后14天,病灶內(nèi)壞死的腦組織大部分由巨噬細(xì)胞代替,術(shù)區(qū)腦組織血管增生明顯;術(shù)后21天,殘腔邊緣銳利,腦組織水腫明顯減輕,炎性細(xì)胞明顯減少;術(shù)后28天,術(shù)區(qū)周邊腦組織水腫基本消失,血管數(shù)量未見明顯變化,趨于正常腦組織。(2)免疫熒光組織化學(xué)染色:a術(shù)后7天、14天、21天、28天組術(shù)區(qū)周邊腦組織可見數(shù)量不等的新生血管,且均高于正常對照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后7天組新生血管數(shù)最多,大于其余手術(shù)各組及對照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后1天組與對照組差異無統(tǒng)計(jì)學(xué)意義(P0.05);b術(shù)后28天,術(shù)區(qū)周邊腦組織成熟血管數(shù)量最多,高于其余手術(shù)各組(P0.05);術(shù)后21天高于術(shù)后14天(P0.05);術(shù)后7天與14天之間差異無統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后7天高于術(shù)后1天(P0.05)。c血管成熟指數(shù):術(shù)后28天高于其余手術(shù)各組(P0.05);術(shù)后21天高于術(shù)后14天(P0.05);術(shù)后7天、14天之間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。d術(shù)后各組術(shù)區(qū)周邊腦組織細(xì)胞密度與對照組比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),術(shù)后各組間比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。3.腦組織切除術(shù)后,術(shù)區(qū)周邊腦組織ADC值變化與細(xì)胞密度無相關(guān)性(相關(guān)系數(shù)r=0.17,P0.05)。結(jié)論1.術(shù)區(qū)周邊腦組織ADC值變化與術(shù)后時(shí)間具有相關(guān)性,其病理學(xué)基礎(chǔ)為腦水腫、新生毛細(xì)血管增多及成熟、星形膠質(zhì)細(xì)胞增生等。2.術(shù)區(qū)周邊腦組織FA值變化與術(shù)后時(shí)間具有相關(guān)性,其病理學(xué)基礎(chǔ)為神經(jīng)纖維束破壞、血管源性腦水腫、血管改變引起的血液灌注。3.腦組織術(shù)后,術(shù)區(qū)周邊腦組織ADC值變化與細(xì)胞密度無相關(guān)性。4.術(shù)區(qū)周邊腦組織新生血管數(shù)量和血管成熟度變化與術(shù)后時(shí)間具有相關(guān)性。
[Abstract]:Objective to establish the normal rat brain tissue after partial hepatectomy model at different time points around the operated area of MRI brain tissue were observed after operation (magnetic resonance, MR) diffusion weighted imaging and diffusion tensor imaging characteristics and evolution of the law, and to explore the relationship between the changes and pathology.
