【摘要】：目的總結(jié)子宮角妊娠與輸卵管間質(zhì)部妊娠聲像圖特點(diǎn)及鑒別診斷意義。方法對(duì)手術(shù)及病理檢查證實(shí)的48例子宮角妊娠和41例間質(zhì)部妊娠的聲像圖進(jìn)行回顧性分析。結(jié)果子宮角妊娠與間質(zhì)部妊娠聲像圖均表現(xiàn)為妊娠囊或包塊位于一側(cè)子宮角部,或靠近子宮角部的附件區(qū),二者在包塊與子宮角之間是否有切跡、包塊與子宮腔是否相鄰及是否存在間質(zhì)線征的表現(xiàn)上比較,差別無統(tǒng)計(jì)學(xué)意義(P0.05);而子宮角妊娠包塊周邊具有完整肌層組織包繞的發(fā)生率要多于間質(zhì)部妊娠(χ2=5.27,P0.05)。超聲引導(dǎo)或腹腔鏡監(jiān)視下清宮術(shù)獲得成功的患者,其包塊周邊有略厚而完整的肌層組織包繞,較切開取胚、子宮角或間質(zhì)部切除的患者多見(χ2=15.63,P0.05)。結(jié)論子宮角妊娠與間質(zhì)部妊娠超聲主要鑒別點(diǎn)為妊娠包塊周邊是否有完整的肌層包繞,包塊周圍有略厚的肌層包繞是選擇清宮術(shù)主要指標(biāo)之一。
[Abstract]:Objective to summarize and interstitial tubal pregnancy sonographic features and differential diagnosis of cornual pregnancy. Methods of surgery and pathological examination confirmed 48 cases of cornual pregnancy and 41 cases of interstitial pregnancy were retrospectively analyzed. The sonographic results of cornual pregnancy and interstitial pregnancy sonogram showed gestational sac or bag piece is located on the side of the horn of uterus, uterus or near the corner of the attachment area, whether the two notch between the mass and the uterine horns, the mass and the uterine cavity is adjacent and whether there are more interstitial line sign performance, the difference was not statistically significant (P0.05); and the uterine horn pregnancy bag piece the surrounding muscle tissue has a complete package of more than around the incidence of interstitial pregnancy (2=5.27, P0.05). Ultrasound guided curettage or laparoscopic monitoring the success of the patients, the masses surrounding muscle tissue wrapped slightly thick and intact, was cut open to take the embryo, The patients with hysterectomy or interstitial resection were mostly seen (chi 2=15.63, P0.05). Conclusion the main point of ultrasound diagnosis of uterine horn pregnancy and interstitial pregnancy is whether there is a complete muscular layer around the pregnant mass. There is a thick muscular layer around the package, which is one of the main indicators of curettage.
