【摘要】：目的通過(guò)經(jīng)顱多普勒超聲(transcranial Doppler,TCD)對(duì)單側(cè)重度頸內(nèi)動(dòng)脈狹窄(internal carotid artery stenosis,ICAS)的患者進(jìn)行眼動(dòng)脈血流的檢測(cè),觀察眼動(dòng)脈血流與眼部缺血性疾病及頸內(nèi)動(dòng)脈狹窄程度的關(guān)系。方法將經(jīng)頸動(dòng)脈彩色多普勒超聲檢查確診為單側(cè)重度ICAS的31例及頸內(nèi)動(dòng)脈完全閉塞的19例患者納入研究。利用TCD檢查ICAS側(cè)眼動(dòng)脈血流方向、收縮期峰值血流速度(peak systolic velocity,PSV)及阻力指數(shù)(resistance index,RI),對(duì)比分析眼動(dòng)脈逆向血流(reversed ophthalmic artery flow,ROAF)患眼及對(duì)側(cè)眼眼動(dòng)脈PSV及RI值,同時(shí)對(duì)眼動(dòng)脈血流方向與眼部缺血以及ICAS程度的關(guān)系進(jìn)行分析。結(jié)果經(jīng)TCD檢查發(fā)現(xiàn)ROAF 24眼,其中單側(cè)重度ICAS者9眼,頸內(nèi)動(dòng)脈完全閉塞者15眼,ROAF在頸內(nèi)動(dòng)脈完全閉塞組更易出現(xiàn),差異有統(tǒng)計(jì)學(xué)意義(χ2=11.76,P=0.001)。24例ROAF患者的阻塞側(cè)PSV值為(38.68±11.58)cm·s-1,與健側(cè)的(33.53±2.74)cm·s-1相比差異有統(tǒng)計(jì)學(xué)意義(t=29.73,P=0.000);阻塞側(cè)RI值為0.60±0.10,與健側(cè)的0.78±0.10相比差異有統(tǒng)計(jì)學(xué)意義(t=-6.20,P=0.000)。24只ROAF眼中58.3%有明確的眼部缺血體征,而26只正流患側(cè)眼中該比例為15.4%,ROAF組患眼更容易出現(xiàn)眼部缺血體征(χ2=9.99,P=0.002)。利用Logistic回歸預(yù)測(cè)ROAF與眼部缺血的關(guān)系:回歸系數(shù)為0.211(χ2=12.26,P=0.000),OR值為1.235。結(jié)論當(dāng)出現(xiàn)ROAF時(shí),提示重度ICAS甚至完全閉塞;ROAF的流速越快,提示眼部出現(xiàn)缺血性病變的幾率越大。
[Abstract]:Objective to investigate the relationship between ophthalmic artery flow, ocular ischemic disease and internal carotid artery stenosis in patients with unilateral severe internal carotid artery stenosis by transcranial Doppler Doppler echocardiography (TCD).Methods 31 patients with unilateral severe ICAS and 19 patients with complete occlusion of internal carotid artery were included in the study.TCD was used to examine the direction of blood flow, peak systolic velocity-PSV (peak systolic velocity-PSV) and resistance index (RI) in ICAS.
