【摘要】：目的:探討艾滋病合并肺部卡波西肉瘤的CT表現(xiàn)及特征。方法:回顧性分析19例艾滋病合并肺部卡波西肉瘤患者的臨床及胸部CT表現(xiàn)。結(jié)果:19例艾滋病合并肺部卡波西肉瘤患者中,發(fā)現(xiàn)雙肺內(nèi)多發(fā)結(jié)節(jié)影16例,多發(fā)斑片狀實(shí)變影8例;其中肺內(nèi)結(jié)節(jié)影均沿支氣管血管束分布。雙肺支氣管血管束增厚9例,同時(shí)合并小葉間隔增厚。縱隔淋巴結(jié)腫大10例,9例發(fā)現(xiàn)少量胸水,均累及雙側(cè)胸腔。19例患者均未發(fā)現(xiàn)病變累及段以上支氣管內(nèi)壁。17例患者發(fā)現(xiàn)卡波西肉瘤時(shí),體內(nèi)CD4+T淋巴細(xì)胞計(jì)數(shù)低于100個(gè)/UL。結(jié)論:艾滋病合并肺部卡波西肉瘤患者胸部CT表現(xiàn)以雙肺各葉多發(fā)結(jié)節(jié)影和斑片狀實(shí)變影多見(jiàn),肺內(nèi)結(jié)節(jié)影多沿支氣管血管束分布;斑片狀實(shí)變影形態(tài)多不規(guī)則,類似火焰狀為其特征性改變。
[Abstract]:Objective: to investigate the CT features of AIDS complicated with Kaposi's sarcoma.Methods: the clinical and chest CT findings of 19 AIDS patients with Kaposi's sarcoma were retrospectively analyzed.Results among 19 AIDS patients with Kaposi's sarcoma of the lung, 16 cases were found to have multiple nodules in both lungs and 8 cases to have multiple plaques of consolidation, in which pulmonary nodules were distributed along the bronchovascular bundle.Double lung bronchovascular bundle thickening was found in 9 cases, accompanied by interlobular septal thickening.A small amount of pleural effusion was found in 10 patients with mediastinal lymphadenopathy and 9 patients with mediastinal lymphadenopathy. None of the 19 patients with mediastinal lymphadenopathy and bilateral pleural cavity were found with Kaposi's sarcoma. When Kaposi's sarcoma was found in 17 patients, the CD4 T lymphocyte count was less than 100 / UL.Conclusion: the chest CT findings of AIDS patients with Kaposi's sarcoma of the lung are more than those of multiple nodules and patches of the lobes of the lungs, and the nodules in the lungs are distributed along the bronchovascular bundles, and the patchy solid shadows are irregular in shape.Similar flame shape is its characteristic change.
