【摘要】：正患者,男性,61歲,因“上腹部隱痛不適兩周”入院。體檢:皮膚粘膜無黃染,全身淺表淋巴結(jié)均未觸及腫大。腹軟,劍突下可觸及一腫塊,質(zhì)稍軟,有輕壓痛,不易推動(dòng),邊界尚清。實(shí)驗(yàn)室檢查:甲胎蛋白(AFP)、癌胚抗原(CEA)、糖類抗原199(CA-199)均在正常范圍以內(nèi)。B超:肝左葉與胰腺之間見分葉狀實(shí)質(zhì)性團(tuán)塊,腹腔動(dòng)脈及肝總動(dòng)脈亦被包裹在團(tuán)塊內(nèi)。CT:胰頭區(qū)可見多個(gè)腫塊融合,呈不規(guī)則軟組織影,境界尚清,周邊強(qiáng)化明顯,中央壞死,胰體尾未見異常。見圖1。
[Abstract]:The patient, male, 61 years old, was admitted to hospital because of "pain or discomfort in the upper abdomen for two weeks." physical examination: no yellow stain on the skin and mucosa, no swelling in the superficial lymph nodes of the whole body, soft abdomen, a soft mass under the xiphoid process, slight tenderness, and not easy to push. The boundary is still clear. Laboratory examination: AFP, CEA, CA-199are within the normal range. B Ultrasound: lobular solid masses are seen between liver Zuo Ye and pancreas. The celiac artery and common hepatic artery were also encapsulated in the mass. CT: multiple masses were seen in the head of pancreas, showing irregular soft tissue shadow, clear boundary, obvious peripheral enhancement, central necrosis, no abnormality in the body and tail of pancreas, see Fig. 1.
