DWI定量評價非壓迫性神經根炎微創(chuàng)治療價值的動物實驗研究

【摘要】:目的探討擴散加權成像(DWI)技術對腰椎非壓迫性神經根炎不同微創(chuàng)治療術后變化定量評價的價值。方法中國小型豬12只CT導向下非壓迫性神經根炎造模成功14天后,根據(jù)治療方法不同隨機分為4組。分別于實驗豬的L2~3、L3~4及L4~5雙側神經根旁注入相應物質進行治療。每組分別于術前、術后3天、7天及15天對實驗豬進行MRI掃描,掃描完成后處死1只實驗動物,取相應雙側神經根行免疫組織化學檢查。分析各組于術前、術后3天、7天、15天神經根表觀擴散系數(shù)(ADC)值及神經根CD8陽性T淋巴細胞計數(shù)評分的變化,以及兩者之間的相關性。結果 A、B、C、D組治療前、治療后3天、7天、15天神經根平均ADC值及神經根淋巴細胞計數(shù)評分均呈下降趨勢(P0.05);A、B、C、D組神經根ADC值與神經根淋巴細胞計數(shù)評分均呈正相關。結論不同治療方法均能使神經根炎得到較好地改善,DWI技術能對腰椎非壓迫性神經根炎微創(chuàng)治療術后變化進行定量評價。
[Abstract]:Objective to evaluate the value of diffusion-weighted imaging (DWI) technique in the quantitative evaluation of postoperative changes in non-compression neuritis of lumbar vertebrae after different minimally invasive treatment. Methods after 14 days of successful modeling of non-compression neuritis under CT guidance in 12 Chinese miniature pigs. According to the different treatment methods, the pigs were randomly divided into 4 groups. The corresponding substances were injected into the bilateral nerve roots of the experimental pigs, respectively. The MRI scans were performed on the experimental pigs before operation, 3 days after operation, 7 days and 15 days after operation, respectively. After scanning, one experimental animal was killed and the corresponding bilateral nerve roots were taken for immunohistochemical examination. The changes of the apparent diffusion coefficient of nerve root (ADCC) and the score of CD8 positive T lymphocyte count in nerve root were observed on the 3rd day and 7th day after operation, and the correlation between them was also found. Results before treatment, there was no significant difference between the two groups. The average ADC value of nerve root and the score of lymphocyte count of nerve root were decreased on the 3rd day and 7th day after treatment. Conclusion the ADC value of nerve root and the lymphocyte count score of nerve root in group P0.05AZB CU D are positively correlated with the score of lymphocyte count of nerve root. Conclusion the different treatment methods can increase the number of nerve root lymphocytes. The improved DWI technique can be used to quantitatively evaluate the postoperative changes of non-compressed lumbar spondylolitis after minimally invasive treatment.
